Waiver of Six Month Advancement Request

Pursuant to 101 KAR 3:045, an unclassified employee’s six-month salary advancement may be delayed by the appointing authority but must be granted no later than the first day following twelve months of service; however, an employee may voluntarily request to waive the six month salary advancement. In that event, this form is available for that purpose.
