Nomination Form for Agency Use

Eligibility:

·  Nominees that received recognition in the 2014 Awards (winner or finalist) cannot be renominated.

·  All Federal, State, Territory and Local Government employees are eligible to be nominated for these awards.

·  Nominations must have the support of a Senior Executive i.e., Chief Information Officer, General Manager, Chief Executive Officer, Assistant Secretary or other appropriate Senior Executive representative.

A nomination for an agency Chief Information Officer must be supported by the relevant agency Head, Chief Executive Officer or General Manager.

Nomination process:

Agencies may nominate more than one employee/team for these awards.

·  Nominations for the ICT Professional of the Year (Individual and/or Team) & ICT Young Professional of the Year Award (Individual only) are to be submitted by the nominee’s agency.

·  Nominees for the ICT Young Professional of the Year Award must be under 35 years of age before 31December2015.

·  Nominations are to be submitted using the online nomination form provided.
This Word document is provided to assist agencies in preparing their nomination and finalising their approval processes before transferring their nomination details to the online form. The online form will be available from Monday 2February2015 until 6pm Tuesday3March2015.

·  All sections of the nomination form are mandatory and must be completed.

·  All key points should be made on the nomination form as judging will be based primarily on that material.

·  One attachment providing associated website links, screen shots and supporting images
(maximum of 2single sided A4pages) is permitted.

Please do not use the attachment to provide additional text in support of your nomination as it will not be forwarded to the judging panels.

·  Confirmation of the receipt of a nomination will be automatically generated on submission of your online nomination.

·  Remember nominations close 6pm Tuesday 3March2015.

Privacy:

The information contained in the online nomination form will be held in confidence and is subject to the provisions of the Privacy Act 1988. The administrator of these Awards is the Department of Finance (Finance). Finance is bound by the Information Privacy Principles of the Privacy Act.

The information contained in the nomination form is collected to enable Finance to assess the merits of a nomination and to contact all parties relevant to that nomination.

Details of the nominations are only accessible to the Award Coordinators at Finance and the judging panels.

How nominations will be judged:

The judging process will be undertaken in two stages.

Stage 1:

A Reference Panel will assess all nominations and recommend a short list of nominees to the Awards Judging Panel. The Reference Panel is comprised of government and industry experts appointed by Finance.

Stage 2:

An Awards Judging Panel will assess the shortlisted candidates in accordance with the Award criteria and determine the winner. The Awards Judging Panel is comprised of state and territory Chief Information Officers and independent ICT industry representatives.

The final decision to shortlist any nomination or to confer any award is based on there being nominations of an appropriate standard. The judging panel may decide not to confer an award if the nominations are not of a sufficiently high standard. All decisions and recommendations made by the panels are binding and final and no discussion or correspondence will be entered into.

Award presentation:

The Awards presentation will take place in Sydney at the CeBIT Gala Dinner & Awards night, at Sydney Olympic Park, Homebush, on Tuesday6May2015. All shortlisted nominee/s will receive an invitation to the dinner.

SELECTION CRITERION
The sole criterion for these Awards is outstanding service and professionalism in the ICT arena.
“Outstanding service” includes service above and beyond the normal requirements of the position, a special achievement of success in the performance of duty in difficult or unusual circumstances, or sustained high level performance by an individual (or team) with a focus on outcomes and recognisable benefits to clients and their workplace.
