  1. Kindly indicate if the financial statements of the Provincial Board of the Community Policing Forum indicated a shortfall and if so
  1. Provide the amount,
  2. Indicate if an investigation was launched to determine the reasons for the shortfall and if not,
  3. Why not,
  4. Indicate if a case was reported at the SAPS and if so,
  5. Provide the case number,
  6. The police station where it was reported,
  7. The date of such report and if not
  1. Kindly provide the reasons for not doing so.
  2. Who is the accounting officer responsible for the Provincial Board of the Community Policing Forum?

  2. Please provide the statistics on the numbers of people in this community who made use of this clinic.
  3. Are there any plans afoot to re-open this clinic, if not, why not, if yes, when?
  4. Where the people in this community are referred to for treatment and how far is it from their current location.

  2. How many SAPS members are in this team.
  3. Kindly indicate from where did this team operate and what provision were made for the accommodation of the team.
  4. What was the monthly accommodation costs including meals for this team for every month since its inception.
  5. How many vehicle were allocated to the team and how many kilometers were logged for each this team report.
  6. To who does this team report
  1. For how long is in envisaged that this team will still be required to operate.
  2. Kindly indicate when assessments were done for the continuation of this team and who approved the continuation in each instance.

QUESTION POSED BY HON. FRANZSEN TO THE MEMBER OF EXECUTIVE COUNCIL RESPONSIBLE FOR LOCAL GOVERNMNET AND TRADITIONAL AFFAIRS [091027-W62]

For the North West Province, indicate per Municipality, per political party and name of Councillor;

  1. All outstanding monies owed to the Municipality by Councillors.
  2. Indicate as above for each Councillor the category (rates, electricity, services, etc)of the debt,
  3. Indicate as above the periods and values per period of any outstanding debt.
  4. Indicate as above detail of any arrangements entered into with Councillor for outstanding debts,
  5. Indicate as above if any loans have been granted to any Councillor and for what purpose and the under what terms.

Lastly, in terms of the Local Government Systems Act, Schedule 1 (code of conduct for councilor) clause 12 a

  1. What actions are planned to recoup any outstanding monies not allowed as per the Systems Act as above, and
  2. Is action planned against any official/politician that allowed any Councillor that may owe monies contrary to the System Act, and
  3. If not, why not, and
  4. In terms of schedule 1 clause 14, has any action taken against councilors or is any action envisaged?

  1. Whether there are any vehicle testing stations that have been closed, if so where and when in respect of each station?
  2. Why have they been closed and whether consideration has been given to how citizens within the jurisdiction of those stations will be assisted to access service?
  3. If human error is the cause of such closure, who are those involved and how have they been or will be dealt with?
  4. Will he on a regular basis keep the Legislature and the public informed about the progress of the cases?

  1. How many motorists have been issued traffic tickets for using cellular phones while driving for the period January, 2009 to August 2009 – indicate by policing district?
  2. How many, if any, have (i) paid the fines (ii) opted to go to court and in the latter event, what have been the findings?
  3. How many motorists per district have been arrested for exceeding the speed limit in (i) built up area and (ii) provincial and national roads?
  4. How have the cases been resolved?

With reference to The Human Settlements Minister, Tokyo Sexwale’s statement that there are 1500 houses that must be destroyed in the North West kindly indicate

  1. The number of the identified houses on each town
  2. And in each instance
  3. The contractor(s) involved
  4. Whether the costs of demolishment and rebuilding will be recovered from the contractor
  5. If the NHBRC was involved in each of these incidents and if not specify why not
  6. Specify the specific localities of their involvement and
  7. The extent of their involvement
  8. What steps have been implemented to prevent a re-occurrence?

ELECTRICITY KOSTER HOSPITAL

  1. Kindly provide details of the origin of electricity supply problems experienced at the Koster Hospital.
  2. Please provide statistics for the last year(2009) pertaining to the number of power failures experienced at this hospital – in each instance provide the date and duration of the incident.
  3. When last was maintenance carried out on the back-up generator(s) at the hospital and give reasons as to why it was not functioning during the electricity failures.
  4. Who is responsible for the maintenance of the back-up generators.
  5. Has this hospital experienced any fatalities due to power failures, if yes, please provide the necessary statistics and dates and names of patients.
