ATTACHMENTS: Please provide the Foundation with a final financial statement related to this project that includes all funders, revenues, and expenses. Please include copies of all receipts for items or work valued at $3000 or greater per expenditure.

It is imperative that your organization has utilized these funds for the project as described in your application. In the event that your project came in under budget by a margin of 5% or greater, or if your project was unable to be completed, you MUST return grant monies to us immediately. If your project is delayed in any manner, you are required to contact us right away to discuss your new timelines and the possibility for an extension.
