An opportunity has arisen for suitable applicants to play a key role in the continuing development of the Dumfries and Galloway Out of Hours Service.
The service operates between Dumfries and Galloway Royal Infirmary in Dumfries and the Galloway Community Hospital located in the West of the region in Stranraer. We provide a service for a resident population of approximately 150,000 people with a significant seasonal tourist population who come to enjoy the amenities of this beautiful area in South West Scotland.
We are replacing and adding to our core team of Salaried GPs who provide the foundation of the service and are complemented by a team of sessional general medical practitioners. You will be involved in delivering unscheduled primary care services between the hours of 6.00pm and 8.00am on weekdays, throughout the weekends and on public holidays.
As a key medical practitioner, you will participate in development activity and projects within the Out of Hours Service and will play a lead role in the integration of the Health Board’s Out of Hours and Accident and Emergency Departments. You will enjoy access to protected time for Continued Professional Development within an environment that offers variety and real scope for innovation. There are close post-graduate and under-graduate links with the ScottishMedicalSchools and RoyalColleges.
Dumfries and Galloway has an excellent reputation for its high standards of general practice which has been demonstrated by our practices achievements in the Quality and Outcomes Framework of the GMS contract.
You will have previous experience of both in and out of hours services and have a facilitative and supportive style of working. Successful applicants will hold a Certificate of Prescribed Experience from JCPTGP or equivalent with RCGP membership being advantageous.

2 Job Purpose
The salaried GPs will provide care, establish and maintain quality standards, develop and initiate new systems of working and encourage and promote the development of clinical governance. Salaried GPs work as part of a multi-disciplinary team to provide a range of out of hours services within Dumfries and Galloway.
3– Organisational Position
Out of Hours GP’s will report directly to the Clinical Director and will not have any line management responsibility. Organisational Chart below:
4Scope and Range
Therange of Out of Hours work is the range of urgent and emergency conditions typically seen in general practices. While there will be some joint working with the Accident & Emergency Department, doctors for the OOH’s service will not be expected to routinely see trauma or A&E type cases.
5. Main Duties and Responsibilities
  1. Provide, as part of a core team of Salaried GPs, Nurse Practitioners, Community Nursing Staff and Paramedics a high quality of Out-of-Hours Service to the public within Dumfries and Galloway in line with local and national priorities.
  2. Provide appropriate clinical management of all patients referred to the service.
  3. Where clinically required attend home visits.
  4. Occasional requirements to provide local triage, consultation and appropriate management of patients by telephone.
  5. Develop and encourage positive working relationships with local general practices, housing, social work, voluntary services, drug and alcohol, mental health, dental, secondary care and other appropriate services.
  6. Utilise all primary care and social work services and facilities to manage patients where clinically appropriate within the community rather than to admit to secondary care.
  7. Occasional requirement to attend patients (with medical problems) in community hospitals and other NHS in-patient units.
  8. Keep contemporaneous, accurate and legible/electronic records of all patient contacts and contribute to electronic data monitoring and audit as directed by the service within the regulations of the Data Protection Act (1998).
  9. Participate / contribute in operational and strategic planning and development of the out-of-hours centres where appropriate, and further development of the service.
  10. Ensure continuing professional and personal development.
  11. Participate / contribute in development activity and project within the out of hours services. Including development/review of protocols, documentation, clinical governance, audit activity, training, research and liaison with NHS 24.
  12. NHS24 will provide the triage service, however in the event of service failure, GP’s would be asked to assist until the service resumes
  13. Participate in the education and training of students and other professionals as required.
  14. Responsibility for routinely participating in audit and research. You will have a key role in participating in and influencing the strategic development of the out of hours service in Dumfries and Galloway.
  15. Contribute to clinical governance standards ensuring service encompasses the standards

The Primary Care Centre will be fully equipped for the purpose.
Systems used by the GP’s include Adastra. Adastra is a computerised call management system designed to operate specifically in out-of-hours organisations. It is an easy-to-use tool for electronically recording action taken during an out-of-hours consultation. It links directly with NHS24 and provides an audit log detailing actions and timings on a particular call. Current consultation information available includes history, assessment of the triage nurse, allergies and medication information.
7Decisions and Judgements
The post holder will be self-directed within agreed objectives as set by the Clinical Director/Medical Director or Deputy.
Please note that GP’s will be covered by Crown Indemnity in all instances when all necessary protocols have been followed when working for the service
8Communications and Working Relationships
Internal:
The Out of Hours Service Management Team, Medical Director, Clinical Director, Consultant in A&E, Local GPs, Nursing staff, all Managers and Clinicians across NHS Dumfries and Galloway, Member of Primary Care Teams, Secondary Care Clinicians, Senior Managers and Clinicians, Board Members, Other OOH Staff.
External:
External agencies such as NHS 24, the Scottish Ambulance Service, the Social Work Department, the Police and the General Public
Communication will normally be verbal via face to face consultations with patients, and direct liaison with colleagues in relation to care provision. However there will also be a requirement for written reports, audits and evidence to demonstrate compliance with relevant standards as required.
The majority of this communication will be with the general public in delivering the care and other OOH’s care providers i.e. in general every shift that is worked will include this form of communication.
9 Physical Demands of the Job
GP’s will be required to potentially visit patients across the geographical spread Dumfries and Galloway during a shift. However, transport and a driver will be provided to drive the GP to required locations. If not carrying out home visits, GP’s may be seeing patients within the Primary Care Centre.
9Most Challenging / Difficult Parts of the Job
  • Call prioritisation (triage undertaken by NHS24)
  • Ensure high quality of patient care with a wide range of problems are dealt with in a timely manner in association with others (e.g. mental health teams, social work, hospital staff, district nursing service and specialist palliative care teams).

11Knowledge, Training and Experience Required to do the Job
  • Required qualifications include a Certificate of Prescribed Experience from JCPTGP
  • Membership of the RCGP would be ideal
  • The postholder will be subject to both informal review and formal annual appraisal provided by the Primary Care Organisation. NHS Dumfries and Galloway will provide medical indemnity.
  • Post specific training will also be provided and should be maintained on a regular basis i.e. update of resuscitation skills
