2007 Cheerleading Season

Dear Newtown Nighthawks Parents/Guardian,

Welcome to the 2007 season. In your registration packet you will find the forms listed below that need to be filled out and returned before your child can participate in Cheerleading. The following is a brief description of the forms and requirements:

  1. 2007 Registration Form (Completed and turned in with your registration fee).
  2. It is very important that you include your e-mail address on the registration form, as this will be our primary source of communication.
  3. 2007 Calendar
  4. Please take a moment to review the schedule of commitment and make sure that any other arrangements/clubs/activities do not interfere with this schedule.
  5. 2007 Rules and Regulations Contract
  6. Please read the 2007 Rules and Regulations with your cheerleader. Both parents/guardians and cheerleader need to sign and return it as part of registration.
  7. Parent/Guardian Behavior Contract
  8. Please read the Parent/Guardian Behavior Contract before signing and returning.
  9. Copy of your child’s Birth Certificate
  10. Please make a copy of your child’s birth certificate and submit it.
  11. Returning cheerleaders from 2006 do not need to submit one, I have the copy from last year.
  12. Teams that qualify for Regional/and or National Competition will be required to submit an original birth certificate to Newtown Youth Football/Cheer Association. It will be returned after competition.
  13. Copy of your child’s 2006/2007 Report Card (FULL YEAR)
  14. As soon as you receive your child’s report card in June, please make a copy of it and mail it to me. Partial year or one semester will not be accepted.

  16. Use you child’s legal name as it appears on the birth certificate when filling out this form.
  17. One of the most important sections of this form is the DOCTORS CONSENT. National rules require that your child have a physical exam after January 1, 2007, so schedule one now and get it completed and signed by your child’s physician. The doctor's signature and stamp are BOTH required.
  18. Please do not attach your own photo to the registration form. The Official Validation photo will be taken on the uniform fitting day.
  19. Payment Information and Form Return
  20. Cost per cheerleader is $155.00 + Uniform Package (See Rules & Regulations IV. 1)
  21. Please make checks payable to: Newtown Youth Football/Cheer Association
