APPLICATION FOR DPD VERIFICATION
A VERIFICATION STATEMENT is provided to B.S. degree students who complete the undergraduate Didactic Program in Dietetics (DPD) courses at New York University. Graduate students who do not have prior verification from an Academy of Nutrition and Dietetics accredited program are also eligible for DPD Verification from NYU if they have taken a minimum of 15 NYU credits.
If you are a graduate student and you did NOT do your undergraduate degree at NYU, you are required to pay a processing fee of $25. Cash or check (made out to the department) is acceptable (see bottom of this page).
If we do not have one of your official transcripts from another school, the evaluator will contact you and YOU must request it from the school you attended and provide it to the evaluator. If you are currently enrolled in a course at another institution, please make a note on the bottom of this form indicating the institution and class. Make sure to send us an official copy of your transcript once your grade has been posted.

PLEASE ALLOW 3 WEEKS FOR COMPLETION OF THE PROCESS. A COMPLETED LETTER OF INTENT OR VERIFICATION IS ONLY GUARANTEED IF THE APPLICATION IS SUBMITTED 3 WEEKS PRIOR TO WHEN YOU NEED IT.
