New HCS Contract for Roads to Community Living (RCL) Demonstration and Washington Roads Providers: Specialized Transition Goods and Services (Assistive Technology/ Vehicle Modification/Service Animal)
Purpose: / To issue a new contract for specific RCL demonstration and WA Roads services.Background: / In 2007, DSHS was awarded a “Money Follows the Person” grant from the federal Centers for Medicare and Medicaid Services (CMS) for the “Roads to Community Living” (RCL) demonstration project. The purpose of the RCL project is to examine how best to successfully help people with complex long-term care needs transition from institutional to community settings.
As part of the Money Follows the Person grant’s demonstration, new services were developed to assist clients transitioning out of institutions.
Washington Roads is a state-funded program which provides pre-transition assistance and support to individuals residing in nursing facilities who wish to relocate to the community and who are not eligible for RCL. Many WA Roads transitional services are provided by the same contractors who provide demonstration services to RCL clients.
What’s new, changed, or
Clarified / HCS has developed the Specialized Transition Goods and Services (Assistive Technology/ Vehicle Modification/Service Animal) Contract for the following transitional goods, items or services:
Assistive Technology (AT) and Vehicle Adaptations
· SSPS Code 5892: Purchase of assistive technology or vehicle modifications that are used to increase, maintain, or improve the functional capabilities of participants. (Available for clients on RCL and WA Roads.)
Service Animal
· SSPS Code 5890: Service Animal: Payment to an RCL contracted Service Animal provider for services which may include (available for clients on RCL only):
o The provision of a trained service animal
o Specific training of the service animal to meet the individualized needs of the participant.
o Orientation of participant with service animal
o Service animal training with participant
o Consultation with participant as needed
When necessary to help a client transition out of the nursing facility, AT and Vehicle Modification services may be provided to WA Roads clients by authorizing WA Roads Specialty Transition Preparation services (SSPS code 5602) or WA Roads Assistive Technology (SSPS Code 5603). WA Roads code 5602 can only be authorized while the client is in the nursing facility. Authorized WA Roads services must not be duplicative of services provided by the nursing facility.
ACTION: / Begin using the new Specialized Transition Goods and Services (Assistive Technology/ Vehicle Modification/Service Animal) contract available in the Agency Contracts Database (ACD) for providers performing the services listed above. Contracts are executed through the AAA unless other local agreements are in place that state otherwise.
Contracting staff must choose the appropriate contract subcode(s) in the ACD which will populate the contract with the applicable Statement(s) of Work under subheading “f” (see subcode information below). Contracts will be available in the ACD after final revisions are made and the final MB is distributed.
As part of the contracting process, qualifications must be verified.
