This form is also to be completed by home-schooled participants who are unable to provide proof of Academic fitness as required by the NHYFSC.

Dear School/Home School Administrator:

The New Hampshire Youth Football & Spirit Conference requires that all our participants maintain satisfactory progress in their academic studies. Our goal is to build sound minds and bodies for our leaders of tomorrow.

While we realize not all of our participants may meet our academic standards, we are taking steps to monitor their grades and help them improve in their academic areas of difficulty. We do not want to penalize any child by not allowing them to take part in our program, so we are asking school officials to help us determine if participating in the New Hampshire Youth Football & Spirit program WOULD or WOULD NOT benefit the above-mentioned child.

Please take a moment to fill out the section below. By signing this form, you and/or your school will not be held accountable for the participation of this child. This form will help the local association to monitor the progress of the child throughout the season. This is just a step-in aiding and assisting these children at both the school level and athletic level.
