5.Authorization and Empowerment for Non-Owned Facilities

For each Facility that the intended transferee will not own, identify the owner of the Facility and the manner in which authorization and empowerment from the owner will be demonstrated by section 3.2.2.3 of Market Procedure MP-04.

6.Connection Agreement(s)

For each Facility, identify all applicable Connection Agreements required by the Market Rules by date and identity of the parties. Where one or more new Connection Agreements will be entered into, indicate the manner in which the existence of each Connection Agreement will be demonstrated and confirm whether each Connection Agreement is in the form required by, and otherwise complies with, the Market Rules. Where one or more existing Connection Agreements will be transferred or assigned, indicate the manner in which the transfer or assignment of each existing Connection Agreement will be demonstrated and confirm whether each transferred or assigned Connection Agreement is in the form required by, and otherwise complies with, the Market Rules.
