Video nomination
This year, we ask that nominations are made by video submission. Nominators can use any type of camera, including mobile phones. Nominators are free to use their imagination in how they present their nominees in the video, as long as they stick to the given time limits. The video should be in a format that can be played on computers, and should be submitted on a USB drive or CD. If you have further questions, please contact Niraj Shrestha.
Nominations acknowledgement
Nominations will be acknowledged by email upon receiving the nomination form and the video.
Finalist announcement
Three finalists in each category will be announced on Wednesday, 11 October 2017. The finalists and their nominator will be informed.

Nomination Guidelines
NDS organisational members or organisational associates can nominate across five awards categories.
Individual Categories
(The nominee has to be an employee of NDS organisational member or associate.)
Outstanding Frontline Worker Award
The Outstanding Frontline Worker Award recognises a frontline worker that demonstrates an outstanding level of service excellence by going above and beyond their role to improve outcomes for people with disability. The nominee should be a person who provides day-to-day service or someone in direct contact with people with disability in a variety of settings.
Emerging Leader Award
The Emerging Leader Award recognises an individual within an organisation who has demonstrated exceptional leadership qualities in their position by taking responsibility, raising awareness, becoming a role model for their colleagues, and improving outcomes for people with disability.
Organisational Categories
(The nominee must be an NDS organisational member or associate.)
Innovative Practice Award
The Innovative Practice Award recognises a disability service provider that demonstrates innovative practice in any aspect of service delivery. The nominee should demonstrate that they have adopted innovative ways to change the service delivery model, and that these have been instrumental in their NDIS readiness journey.
Excellence in Engagement Award
The Excellence in Engagement Award recognises a disability service provider that demonstrates significant engagement with people with disability through marketing, capacity building, and awareness raising activities to help them transition to NDIS.
Excellence in Regional Services Award
The Excellence in Regional Services Award recognises a project or team associated with a disability service provider in South Australia that has made a significant contribution to providing quality support and services to improve the lives of people with disability living in regional and rural South Australia.
Conditions of Entry
· Nominee(s) must be an NDS organisational member or organisational associate.
· Only one nomination per category per organisation is permitted.
· Each nomination requires its own nomination form and video.
· As the nominator, you must seek the approval of the nominee prior to submitting your nomination. The nomination form must be signed by both the nominee and the nominator. By signing the nomination form, both the nominator and the nominee indicate their consent to the Award’s conditions of entry.
· The information provided in the nomination form and the video submission will be made available to the judging panel. By entering the Awards and signing the nomination form, it is agreed that all information included in the submission, as well as photographs from the Awards event, may be used for promotional purposes by NDS.
Judging and Award presentation
· The Awards will be judged on the relevance of the nomination to the Award description and evidence.
· Nominations for the Award categories will be assessed and evaluated by a panel selected by NDS. Judges will be required to declare a conflict of interest and withdraw if they are associated in any way with the nominator, nominee or the nominated program/project.
· The judges’ decision will be final and no correspondence or discussion will be entered into.
· The three finalists in each category will be invited to the ceremony.
· The top three finalists in each category and the Award winner will be announced on Thursday 19 October 2017 at the Adelaide Convention Centre (after the NDS SA State Conference).
Privacy and Confidentiality
The information provided in the nomination form is collected by NDS and may be made available to the judging panel and other agencies contracted for the purposes of the Awards.
