To deliver and develop a physical activity pathway for patients completing their initial treatment within the National Centre of Sport and Exercise Medicine (NCSEM) NCSEM Health Professionals will refer to this role by prescribing exercise as medicine to instigate physical activity within Places for People Leisure NCSEM venues and additional activities to reduce sedentary lifestyle to prevent and manage disease. This role will also work with key managers to engage with the local community to deliver health and wellbeing opportunities delivering key health benefits including sustainable physical activity.

KEY OBJECTIVES

  1. Promote activities and management of NSCEM passes
  2. To be responsible for the quality, consistency, and safe delivery of the service and responsible for the clients experience in all matters of service delivery
  3. Add value to the programmes by continual research and development and through working with colleagues and sharing best practice
  4. Ensure the referral scheme adheres to all relevant national guidance including NQAF for exercise referral
  5. Develop and maintain knowledge of local physical activity opportunities that can form part of clients exit plans
  6. Screen and processing client referral paperwork , gaining additional medical information where required to safely enable clients into exercise
  7. Undertake 1-1’s consultations to enable clients into suitable physical activity around health inequalities and other potential barriers. This may include individually prescribed exercise, signposting into specialist class environments and independent usage of PFPL Health and Fitness facilities. Also signpost to services most appropriate to the health and income needs of the patient
  8. Conduct inductions/orientations for clients for gym programmes, and physical activity interventions to support NCSEM clients including specialist populations. This includes collection of wellness markers to measure impact of the service.
  9. Input all client data onto database software to enable report generation and monitoring of the pathway. Support data both quantitatively and qualitatively to enable research or to develop training and development of health and fitness professionals
  10. Encourage sustainability of physical activity post intervention achieving conversion to in-house memberships/packages and pay as you go options. Support a minimum of 30% of clients onto a paid package after the client’s 12 week complimentary pass
  1. Working in partnership with the Contracts Health and Wellbeing Manager to monitor and evaluate the scheme effectively to show impact of services
  2. Present progress updates and & outcomes to senior PFPL staff and healthcare professional as and when required. Support the evidence base for the integration of health and fitness through collaborative working with the NCSEM Operations Manager and CHWM.
  3. Deliver health specific exercise and support services (qualification specific) as & when required.
  1. Support CHWM Co-ordinate liaise between physios and other Health Professionals onsite with the NCSEM and in the community to actively contribute to partnership initiatives, with the aim of maximising service delivery and effectiveness
  2. To undertake training and continuous personal development as necessary and relevant to the post.
  3. To carry out other related duties and responsibilities as may arise within the scope of this post to support Places For People deliver a flexible health and wellbeing service within Sheffield
