National Provider Identifier (NPI)

To apply online for an NPI number or update your NPI information, go to https://nppes.cms.hhs.gov

Click on National Provider Identifier (NPI) to login or apply for an NPI number.

If you have never had an NPI number, click Apply Online for an NPI. Create a User ID and password. Some guidelines for entering information:

§  Enter information as an Individual (not an organization)

§  It is recommended you use your Sunny Hills Services or BAYC email address

§  Anyone can run a search by first and last name. “Provider Business Mailing Address” and “Provider Business Practice Location Address” will appear as public information. If you do not want your home address made public, do not use home address as a business address.

If you already have an NPI number and need to update your information, go to https://nppes.cms.hhs.gov and click Login. Enter User ID and password. Update NPI information when:

§  You are a SHS new hire and have an existing NPI number with other agency information

§  You change positions and the new position requires an NPI number or a change to your NPI number

§  You have a license change

§  You change from a registered MFTI or ASW to licensed clinician

§  You change from a Master’s level or MHRS status to a registered MFTI or ASW

§  You legally change your name

§  Your discipline changes and you need to update your title, license, or registration number.

Recommended Taxonomy Codes and Taxonomy Descriptions:

§  If you are a licensed MFT select Taxonomy – Behavioral Health & Social Services Providers - Marriage and Family Therapist

§  If you are a licensed LCSW, select Taxonomy – Behavioral Health & Social Services Providers - Social Worker - Clinical

§  If you are a registered MFTI or ASW, select Taxonomy - Behavioral Health & Social Services Providers - Counselor Mental Health

§  If you are an MHRS (Mental Health Rehab Specialist), select Taxonomy - Behavioral Health & Social Services Providers - Counselor

§  If you are a non-MHRS (Mental Health Rehab Specialist), select Taxonomy - Behavioral Health & Social Services Providers – Rehabilitation Practitioner

If you update your NPI number information, you may need to update county staff number information. Use the Staff Number Procedure to update your staff number information.

Contact the NPI Enumerator at 1-800-465-3203 Mon–Fri (Eastern) if you have forgotten your User ID or cannot reset your password.
