  1. All chapters are required to complete these Annual Reports which includes all departments.
  1. The Annual Reportsmay be completed by the president, secretary or any officer as determined by the chapter.
  1. Please note that these forms will be available on the website, ,chapters who wish to complete them online:
  • The online version has been formatted to expand and allow you ample room for providing information
  • Please use a page break between reports
  1. Please have reports typewritten, if possible.Each department report needs to be submitted on a separate piece of paper.
  1. Please forward these reports to your jurisdictional chapter immediately following a meeting held to approve reports, not necessarily the annual meeting,not later than 15 January. Keep a copy for your files.

Chapters under Provincial jurisdiction- please forward2 copies of this report to the Provincial Secretary not later than 15 January
