Participants in the 2012 Taste of Warren Hills serve portions not to exceed 3 oz. The tasting portions will have a selling price each restaurant/vendor sets at $1, $2, $3, $4, or $5. Please include the items your restaurant intends to serve in the form below. Cash may not to be accepted at the booth. Tasting tickets will be sold to patrons for $1 each upon entry as well as by Taste of Warren Hills volunteers throughout the day. Restaurants/vendors will receive 25% of their chosen selling price for each tasting. Taste of Warren Hills event organizers will provide each restaurant/vendor booth with a ticket container labeled with their restaurant/vendor name. It is veryimportant that all tasting tickets collected during the event are placed in the ticket container. The total amount of tickets that are collected by each restaurant/vendor booth will determine the amount of profit (25¢ per ticket) that each participant generates at the event.

Please include a check payable to Warren Hills Sports Association for the entire amount with your application. Mail completed application with payment to: Warren Hills Sports Association, Restaurant Chair Committee, P.O. Box 398, Washington, NJ 07882-0398. Registration deadline is March 30, 2012.
I have read and agree to comply with the Warren Hills Sports Association and the Health Department rules and regulations. I understand that I will be liable for any cost incurred by the Warren Hills Sports Association for any failure to comply with these regulations. I understand that all pre-paid fees are non-refundable. I authorize the Warren Hills Sports Association to deduct 75% of the proceeds of restaurant/vendor ticket sales the day of Taste of Warren Hills. I understand the Taste of Warren Hills is an event that uses event tickets for all purchases and that any participant that accepts cash will be asked to leave immediately.
