The Puyallup Farmers Market Entertainment Stage is devoted primarily to local groups and individuals. If you would like to be considered to perform on this stage, please complete this form and return it along with any promotional materials and CD of your musicto us at least 2 weeks prior to your requested performance date. You can reference a website of your music if a CD is not available.

There are two time slots available for stage performances on Saturdays.Performers must end at the listed time in order for set/equipment changeover or break down. 9 am until 11 am or 12 pm until 2 pm

Entertainment Contract

I have read and understand the 2018 Puyallup Main Street Farmers' Market Rules and are bound by the terms and conditions outlined in the Rules packet. I shall indemnify, keep, and save harmless Puyallup Main Street Farmers' Market, the Puyallup Main Street Association, all Sponsors, and the City of Puyallup from and against any and all claims and demands, whether for injuries to persons, or loss of life, or damage to property, on or off the premises, arising out of the use or occupancy of the premises by vendor and shall defend at my own expense any action brought against the Puyallup Main Street Farmers' Market, the Puyallup Main Street Association, all Sponsors and the City of Puyallup by my acts or omissions.
