As an athlete/athletic parent in the EnterpriseCitySchool’s Athletic program, I/we understand that participation in any sport can be a dangerous activity involving MANY RISKS OF INJURY. I/we further understand that there are serious risks including and not limited to brain damage, cardiac arrest, serious injury to internal organs and to bones, joints, ligaments, muscles, tendons, and other serious injury or impairment to other aspects of the athlete’s general health and well-being. I/we understand that the dangers of participating in sports also include the potentially high cost of medical care and impairment of the athlete’s ability to earn a living, to engage in other business, social and recreational activities, and generally to enjoy life. Recognizing these risks, I/we consent to the participation my/our son/daughter in the sports program offered by Enterprise City Schools. I/we also agree to comply with all rules, regulations, and recommendations of administrators, coaches, athletic trainers and doctors concerning injury prevention and care. I/we hereby grant consent to any and all health care providers designated by Enterprise City Schools to provide my child any necessary medical care as a result of any injury/illness. I hereby agree to follow the rules/regulations established by the Enterprise City Board of Education concerning off-campus activities. I understand that all school rules/regulations are in force traveling to and from, and at said location. I understand that I will be held liable for any damages to any hotel, organization, school, etc. property that are a result of my personal actions. I am aware that I must make-up any and all assignments during the stated time-frames. I have read and understand the student handbook and understand my personal responsibilities.

I hereby agree to release the Enterprise Board of Education, its representatives, agents, servants, and employees from liability for any injury to above-named person, resulting from any cause whatsoever occurring to above-named person at any time while attending this school-related activity, including travel to and from conference, excepting only such inquiry or damage resulting from willful acts of such representatives, agents, servants, and employees.

AHSAA STAR ELIGIBILITY REQUIREMENT

Dear Parents/Athletes:

The Alabama High School Athletic Association (AHSAA) is requiring every student-athlete to complete an on-line sportsmanship program entitled the STAR Sportsmanship Education Program in order to be eligible to compete in athletics. This is a onetime requirement.

Since this is mandatory in order to receive clearance to play, it is essential that this program is completed as soon as possible.

Below are the steps an athlete must take to complete the STAR course. Please follow them completely.

TO ACCESS STAR SPORTSMANSHIP EDUCATION PROGRAM:

Go to:

  • Choose “Student Sign In”
  • Enter STAR Login code STAR01150
  • Select “ENTERPRISE HIGH SCHOOL”
  • Select your current grade – IF IN 7-9 GRADE, SELECT “ADVANCED MIDDLE SCHOOL”
  • Click “Register”
  • Enter your first and last name
  • You will then be taken through a 10 question pre-test, then into the program.

COMPLETION OF STAR PROGRAM:

Upon finishing the program, print out the Completion Certificate and give it to your coach.
