Substantive Change 6: Ownership
Instructions: Complete the Substantive Change Application Cover Sheet and Substantive Change 6 Application electronically and email to . Upon receipt of this application and the corresponding payment, the MACTE office will create a submission report within MACTE’s Accreditation Management Portal to submit all documentation required for the change.
Please remember to pay the application fee. The current MACTE fee schedule, inclusive of payment instructions, can be found on the website, www.macte.org, under the resources tab. The fee for a Substantive Change 6 is a Substantive Change Requiring On-Site Visit. Portal access will not be given until both the application and fee are received.

Sale of institution
Transfer of controlling interest of stock of the institution or its parent corporation
Merger of two or more institutions
Division of one institution into two or more institutions
Transfer of assets that comprise a substantial portion of the educational business of the institutions
Change from non-profit to for-profit
Transfer of ownership based upon illness or death of the primary owner

The items listed below are required when applying for an additional location. These items will be requested in MACTE’s Accreditation Management Portal. Please DO NOT submit this documentation via email with the application forms.

2. Copy of State Post-Secondary Authorization (Date of Application: // )
3. Include a revised Organizational chart
4. Include a narrative description addressing the reason for the change, the process used in accomplishing the change
and the transition between one owner and the other.
