JOB TITLE:Nurse Practitioner

JOB SUMMARY:

The post holder will provide care for the presenting patient through being a strong critical thinker, compassionate and committed whilst working within their professional boundaries to take initial history, carryout clinical assessment, diagnosis, treatment and evaluation of care. They will make professionally autonomous decisions for which s/he is accountable and work within professional codes of practice, guidelines and MMP protocols. The post holder will work within a multi disciplinary team, working in collaboration to meet the needs of patients and the Practice.

KEY RESPONSIBILITIES:

Clinical Practice

  • Make an initial assessment (in physical, psychological and social terms) both by telephone and face-to-face for patients presenting with common symptoms and needs, diagnose where straightforward, and plan, implement and evaluate appropriate treatment in accordance with practicenational guidelines.
  • Seek the advice of a GP principal whenever presentations are complex or fall outside areas of established training and competence.
  • Refer appropriately to community and secondary care colleagues and if necessary after consultation with a GP
  • Order the appropriate clinical investigations and tests to support initial diagnosis
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs andin accordance with evidence-based practice and national/practice protocols and within scope of practice.
  • Work with patients in order to support adherence to prescribed treatment.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote health lifestyles and apply principles of self care.
  • Carry out 6 week childdevelopment checks in accordance with national/ practice protocols.
  • Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation.
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for mental health.
  • Meet the needs of patients presenting for opportunistic minor injury care.
  • Contribute to the Practice achieving its quality targets to sustain the high standards of patient care and service delivery.
  • Help develop and set up new patient services and participate initiatives to improve existing patient services.

  • Practice within policies and guidelines of the NMC and MMP
  • Maintain confidentiality of all patient identifiable information and contacts
  • Maintain and continually develop clinical knowledge and competence
  • Have a clear understanding of the legal issues and professional accountability relevant to the role
  • Maintain skills and competencies in use of the clinical system
  • Inform the Practice Manager or a GP Partner of any professional concerns. Report significant events according to MMP Policy
  • Maintain accurate records of all patient contacts in accordance with NMC and MMP guidelines and best practice
  • Have a working knowledge of Data protection and Caldicott Guidelines
  • Adhere to the MMP complaints procedure
  • Support and contribute to the future development of Practice services

Personal

  • Take personal responsibility for reading and, where appropriate, responding to e-mail communication to and from MMP team and external organisations.
  • Work in a safe manner at all times and to comply with the organisation’s operational health and safety policies and procedures.
