Practical work submission form

This form should accompany all practical assignments (i.e. anything that is not written work). Please copy and paste it or photocopy as needed.

Recordings of performances should be submitted on audio tape, CD, MP3, DVD or video and should be of sufficient quality to accurately represent the performance of the student. Assignments should preferably be submitted as electronic files by email to . The file name should be your student ID, unit number and type of assignment (PA for preliminary assignment and FA for final assignment), e.g. EBJ102008 A22 PA; please use the same code in the title of your email.

Alternatively assignments can be sent by post to RSCM Skills, 19 The Close, Salisbury SP1 2EB. Please clearly mark on the back of your envelope your student ID and assignment numbers.
