How CPN undertake care assessment? - Toinvite Guy Powell for the next meeting ACTION JL

Janet Lloyd has invited Clive Brock from Kent County Council to Carers Consultative Committee meetings to talk about personal budgets

Ivan McConnell mentioneda pilot work in NHS West Kent Clinical Commissioning Group (CCG). West Kent CCG is a body of GPs and other healthcare professionals, responsible for commissioning healthcare services across the West Kent region. They want to feed the patient portal into it and the maim aim of this project is to be able to accessfrom the system, details of an individual andcarers – if agreed, details of the GP, the last quarter recordand medications. This is scheduled for September. PB requested a verbal feedback about the progress in August. IM suggested to invite a representative from CCG and Les Manley to give an update ACTION JL

Dual diagnosis – details were sent to Bobby Taiano and Stephanie Clarke, unfortunately they were unable to attend, to invite them for the next meeting ACTION JL

6.Speaker – Nikki Oatham and Nigel Jacobs

Nikki Oatham (NO) and Nigel Jacobs (NJ) talked about the role of the Emotional Support for Carers. The Trust understands stress which carers experience; they are concerned with how to provide support. Primary Care Psychological Services offers support to carers. NJ highlightedthe role of family practice and involvingall family members in the process. To be able to support this project resources are needed– all the service lines directors will put forward fundsto have a family support worker who will develop training for carers. People can self refer to Counselling and Psychological Services, information can be found on thePatient Portal. ( )

7.Website and the carers’ newsletter – Christina Shaw

Christina Shaw and IM updated group on the new website

Improvements: better navigation, videos on website, easy access from the devices

CS asked carers what they want to improve there. KJ asked for the carer’s booklet, CS explained that this is in the process of updating.Team doesn’t want to duplicate information so instead of duplicating new documents a small amount of information will be provided on the patient portal. They want to encourage people to ask about documents needed.

Carer’s news letter – designed and ready to print, called ‘CONNECTED’ – it is a small way to connect with carers, CS asked for feedback, the idea is to print 500 pages, and note on website if anyone wants to subscribe to the website and receivenews. Carers could relate their stories there, JL said that she has stories which she will be able to share – that will be presented during the conference on 9th June ACTION ALL

Discussion around carer’s forums, to research what kind of forums are around Kent and what kind of psychological support is provided

8.Safe-T, Malcolm Brown

SAFE-T (Suicide Assessment Five-step Evaluation and Triage) presented by Malcolm Brown (MB), Steve Norman (SN) and Chris Fay (CF)

This is a part of a larger thinking about clinical risk. The objective isto transform how people approach clinical risk assessment and management so as to incorporate a totally focused method to safety planning that is fully integrated within the context of a Recovery Model.

Five steps:

  1. Identify risk factors;
  2. Identify protective factors;
  3. Conduct suicide inquiry;
  4. Determine risk level/intervention;
  5. Document assessment, intervention and follow-up.

CF talked about the project within theCommunity Recovery service line, where the presentation and film about safety were presented to staff. Thefilm triggered a discussion around what the team does in the assessment time, how they cope with problems. The Discussion helped to understand the process and problems. Also risk forums were organised. By working together teams could have a better opportunity to reduce a risk.

SN explained that the foundation of safe-t tool is to get people to a basic level - to explain incidents and what to do right, to educatethe team, to work not in isolation but share concerns and learn about them. The process is that all incidents come internally through DATIX, the idea is to have embedded learning for staff and prevent dangerous situations. This will involve listening to the concerns of patients and carers

JL presented the ‘Out of area information sheet’ and ‘Carers Guide to Confidentiality’; both documents are in the process of branding. JK commented that the second document needs to contain information suggested by Dr. Catherine Kinane about what carers should do to escalate their concerns (where to go, what the process is) when the person who deals with them is unable to help or refuses to help. JL will speak to Christiana Shaw (re-print documents or put labels with new information) ACTION JL
