Middle School Student Release Form Following an Away Contest

MIDDLE SCHOOL STUDENT RELEASE FORM FOLLOWING AN AWAY CONTEST

I, ______request that my son/daughter:

______be released from the supervision of his/her coach to my supervision immediately following an away athletic contest. I understand that this allows me to transport ONLY my son/daughter from away athletic contests, and that this relieves the Paradise

Valley Unified School District from all responsibility upon this release. Further, the condition of this release requires that I must personally communicate to the head coach my intention of transporting my student at the time he/she is released.

I agree that Paradise Valley Unified School District will not be held liable in case of an accident during this transportation. I hereby agree, to the fullest extent permitted by law, to release, indemnify and hold harmless the Paradise Valley Unified School District, its officials, officers, employees, representatives, agents, servants, or volunteers, from and against any claims, damages, or liability of any kind or nature for injury, death, or damage to personal property arising out of or in connection with my participation in this activity, from whatever cause, including but not limited to the active or passive negligence of the District, its officials, officers, employees, representatives, agents, servants, volunteers or other activity participants.
