1. Please complete & return this booking form noting invoice address, any Purchase Order Number that needs quoting on the invoice & method of payment.
2. Once this booking form has been received we will send you an invoice.
3. Full and immediate payment is required to secure your place on a theory session.
4. Once payment has been made a confirmation of your requested place will be sent.
Please remember to bring your driving licences with you, and please generate a ‘Check Code’ via dvla.gov.uk (On-line driving licence check) to give to the trainer on the theory day, in order to facilitate driving licence validation

There may be an additional charge if you do not have access to a suitable minibus for the driving/access assessment
Please Note:
MiDAS training is designed to make people aware of their responsibilities and the problems they could face whilst driving a larger vehicle with up to 16 passengers. The Organisation needs to be aware of what their members of staff and/or volunteers can and cannot legally drive.
It is the driver’s responsibility to know what they can and cannot legally drive, & to check in advance of attending a Minibus Driver Awareness Scheme (MiDAS) training course that their driving licence makes them eligible to drive a minibus, & how does this impact on the organisation’s insurance policy? e.g.
· Are there any penalty points and/or disqualifications?
· Is the driver aged under 21 or 70 & above?
· Are there any medical restrictions imposed on their licence;
If a driver does have any driving licence anomalies, this could impact on our insurance policy and the availability of using a suitable minibus to do the driving assessment in.
Organisations should also be aware of the restrictions imposed on any of their staff or volunteers who may be a ‘non D1 driver’ i.e. people who passed their driving test after 1st January 1997.
Non D1 licenced drivers may drive a minibus in UK only but ALL these conditions apply:
a. 21 or over
b. Held full UK driving licence for 2 years
c. Driving on voluntary basis
d. Minibus used by not-for-profit organisation
e. Minibus max weight (MAM or GVW) no more than 3.5 tonnes (4.25 if accessible)
f. Must not tow a trailer
g. Age 70 must pass full ‘PCV’ medical to renew this entitlement.
Cancellation Policy
If a training course is cancelled or postponed at short notice, The Training and Skills Development Foundation reserves the right to charge a cancellation fee based on the price of the training. We will examine each cancellation/short notice postponement on a case-by-case basis take into account any special circumstances surrounding the request for cancellation before deciding whether to invoke a cancellation charge.

Working days are classed as Monday to Friday except for Statutory Bank Holidays, Good Friday, and the period 24th December to 1st January inclusive.
