Dear Student Athlete & Parents,
Once again we will be offering ALL SPORT PHYSICALS in May this year. We want students to take advantage of these free physicals even if they are considering participating in any sport for the 2012 – 13 School year. We have no guarantee that our medical staff will be available over the summer to clear any summer physicals. Therefore those physicals would not be cleared until September. JV, and Varsity Sports Physicals, FOR ALL SPORTS SEASONS, will be conducted after school in the High School Clinic on the following dates.
May 22, 2012 BOYS 2:30
May 24, 2012 GIRLS 2:30
THERE WILL BE NO OTHER DATES FOR SPORTS PHYSICALS OFFERED DURING THE YEAR.
We encourage you to take advantage of these FREE physicals. If you choose to have a private MD perform your physical that form MUST be returned to the respective clinics by June 21, 2012.
IMPORTANT NOTICE:
Without exception-
You will not be able to bring your forms to your coach- they MUST be approved through the Clinic prior to ANY participation.
Both the health history and physical form must be filled out completely, submitted and approved. Your participation is contingent upon the final approval of your forms by Work Fit Medical.
If you receive any injury over the summer, you must bring in your doctor’s release prior to participation.
You will need a pre-scheduled appointment time – don’t delay in seeing the school nurse.
