HOSPITAL CONSULTANTS

It is the surgery policy that all drugs

requested to be started by a SecondaryCare Hospital Consultant will be treated in the same manner as any other prescription request, i.e. they are routine and NOT urgent. Hence this will take the usual minimum time of 48 hours to process and a further 24 hours before the medication is ready to collect from your chosen pharmacy.

It may take longer for the request to be processed if we do not have the full correspondence from the hospital, or if the GP raises a query regarding the medication prescribed.

Under the terms of the local healthcare contract with Western Sussex Hospitals Foundation Trust (St Richards) if a

Consultant wishes a patient to start a new drug sooner than the standard

two week time period, then the Consultant is responsible for prescribing the medication.

REPEAT PRESCRIPTIONS

A repeat prescription can be requested in several ways:

  • byusing a printed or hand written request form, posted into the prescription request box in the reception area.
  • By e-mail to
  • On-line via our website - please ask at reception to register for Online Services.
  • bypost, enclosing an S.A.E.
  • byFax to 01243 530607.

Please allow a clear 48 hours for your repeat medication form to be processed ready for collection at the

reception desk.

If you have requested that the repeat medication form is to be sent directly to a local pharmacy of your choice, including Summersdale Pharmacy, please be aware that it will usually take a further 24 hours until your medication is ready to collect.

We do not take requests for repeat medication over the telephone, as experience shows this can lead to mistakes, something we would all rather avoid.

New NHS Scheme for Patient

Referrals

The aim of this service is to support patients to be treated more quickly, in a place of their choice, closer to home. Patient Choice Navigators will deliver this by the provision of:

  • ‘Choice’ conversations with patients.
  • Being the point of contact for all patient queries regarding referrals.
  • Provision of local knowledge of community and hospital outpatient services.

The new service will deal with the following referrals:

  • Adult urgent and routine referrals to outpatient clinics.
  • Non – obstetric ultrasounds.
  • Direct access echocardiograms.

When a G.P. refers a patient, they will:

  • Advise the patient that they have a choice of where they are treated.
  • Hand the patient a Patient Booking Card which will provide the telephone number of an advisor, the patient needs to speak to the advisor to choose where the referral will go.
  • Tell the patient to call the advisor’s number after five working days.

Should a patient fail to call the Patient ChoiceNavigators they will attempt to call the patient at least three times on different days, however, if after 10 working days no contact has been made, the patient will be allocated the most suitable appointment available, based on treatment waiting times and distance the patient will need to travel.

Staff Training Days

The Surgery will close for staff training on the afternoon of the following dates:

  • Wednesday 28th September 2016
  • Thursday 13th October 2016
  • Thursday 17th November 2016

Patient On-Line Services

Lavant Road Surgery is able to provide on line services allowing you to have access to booking appointments on line, requesting repeat prescriptions and having access to your detailed coded medical record. For further information and to register for on-line services please contact Reception.

Telephone Consultations

When you book a telephone consultation, the GP will generally aim to call you between 11.00am and 2.00pm for a morning appointment and usually from 5.00pm onward for an afternoon appointment.

This is a general guide only and you must be aware that the call may be earlier or later than the given time.

Please ensure that you are available for the

call and that you have left the correct contact details. Also, please be aware that the Caller ID will show up on your mobile phone as

“UnknownCall” “Private Number” or“Withheld Number”

Many patients do not answer this type of call therefore appointments are wasted.

Unfortunately, if you miss the call you may have to re-book on another day.
