For you convenience, we have many affordable options for the uninsured
1) Sick Visit/ Physicals/ Regular care. Patients without insurance may pay per visit. $175.00 for the first visit that includes a baseline physical. Each visit is $125.00 there after. Additional costs may apply for specific treatments; we always try to keep costs down.
2) North Fork Injury Care. Consider us as your on-call provider for non-life threatening injuries. This option is billed per injury and includes an initial assessment visit with one of our healthcare professionals; injury/ complaint specific exam and, as needed, lab work, x-ray, antibiotics, tetanus, supplies, procedure(s) such as suturing or splinting and a follow-up visit. This cost is $399.
3) All patients are eligible for North Fork Complete Care. Our complete care option is for local residents who are seeking a comprehensive health care package. This option covers all aspects of general medical care, including but not limited to:
Annual Physical: Annual physicals are the cornerstones of good health, and they provide our heath care practitioners with a baseline going forward, so this is where we begin our care partnership with you. This will include a focused exam, EKG, labs, urinalysis, and pulmonary function.
Sick Visits: If you have the flu or fever— or just don’t feel well—we will come to evaluate you and provide whatever treatment is needed. This is no limit on the number of sick visits per patient.
Lab work, simple biopsies, and X-ray: All are included in your health care package. Basic labs, blood work, and X-rays often cost people hundreds of dollars but we include then at no additional charge.
Urinalysis: Our mobile unit is equipped with the machine that allows us to provide with immediate results.
Laceration Care (including stitches): You need not go to the Emergency Room for simple stitches! Our practitioners have extensive experience with suturing and our state-of-the-art equipment guarantees that your minor lacerations will be repaired with limited scarring.
Treatment for sprains and minor fractures: We have all splinting material needed to splint sprains and fractures not needing surgery or placement. Crutches or sling will also be provided if needed. Additionally, we have on staff an orthopedic specialist to assist with treatment of orthopedic injuries
Additionally included procedures/ treatments: The above listing is just a sampling; We also provide IV fluid, antibiotic treatments, shave biopsies, abscess drainage, culture of specimens, foreign body removal in eyes, nose and ear—and much more!
The cost of our complete care package is $175 per month for the first patient; additional patients are billed at $150 per month.
*Severe injuries or life threatening injuries should always go to the Emergency Room immediately*
