Media & Internet User Consent Form 2015

Brentwood Primary School staff and students have access to state of the art technologies (computers, digital cameras, digital video cameras) and this gives us the opportunity for students to see their work presented within the school and the wider community. Through this we are able to promote the cultural, artistic, academic and sporting achievements of our students. The following are ways in which your child’s work may be presented within the school or to the wider community.

Your child may have their work published or they may be featured or mentioned:

·  In articles in class or school newsletters

·  In school publications such as year books, annual reports, histories of the school etc, for publication to the wider school community

·  In educational publications such as School Matters for the wider educational community

·  In local, state or national newspapers or other media outlets

·  In web sites promoting the school

Your child’s photograph, individually, or as part of a group, could be published in any of the above. Video clips of your child, individually or as part of a group could be published in any of the above.

Children often watch videos/DVD/television documentaries as part of their learning. Almost always there are ‘G’ rated and don’t require consent. Very occasionally something with a ‘PG’ rating is appropriate for which we would need parental permission.

Local Excursions

Children occasionally walk within the local area for minor excursions under the supervision of the teacher and attend activities in local parks, nature reserves or shopping centre. On all occasions, parents will be notified of the local excursion.

Online Services ACCEPTABLE USAGE AGREEMENT for K-6

Dear Parents/Caregivers

Students at Brentwood are able to benefit from the many advantages ‘online’ resources offer them through the use of the Internet. It is the policy of the school to mandate and expect that all users of such resources do so in a responsible and ethical manner.

Below is the code of conduct governing the use of information and communication technologies and the Internet at Brentwood Primary School. Parents and students are asked to read these and return the agreement to the school. Failure to adhere to this code will result in removal of privileges for a period of time.

This code is the background for students to apply responsible, moral and ethical behaviour to the use of the Internet.

‘On-line’ Code of Conduct

• I will use the school computer only with the permission and direction of a teacher.

• I will follow all instructions when using school information and communication technologies, eg. computers and iPads

• I will not let anybody else know my password.

• I will not let others use my network account unless it is with the teacher’s permission.

• I will not access other people’s network accounts.

• I will only use DET Webmail for school related communications and under the direction of the teacher.

• I know that I am responsible for anything that happens when my network account accessed.

• I will tell my teacher if I think someone is using my network account.

• I am aware that the school and the Department of Education may see anything I send or receive using the Webmail service.

• I will ensure that any email that I send or any work that I wish to have published is polite, carefully written and well presented.

• I will use material from the Internet or other sources only if I have permission to do so. I will not look up unacceptable sites or information.

• If I use material in my work that I have found on the Internet, I will say where it comes from.

• If I see any information that makes me feel uncomfortable I will tell my teacher right away and switch off the monitor.

• I will not reveal personal information, including names, addresses, photographs and telephone numbers of myself or others.

• I will not damage or disable any of the information or communication technologies I use.

• I will not download from the Internet without strict supervision of the teacher.

I understand that:

• I will be held responsible for my actions while using online services and for any breaches caused by allowing any other person to use my network account;

• The misuse of online services may result in the withdrawal of access to services and other consequences dictated in the school’s Behaviour Management policy; and

• I may be held legally liable for offences committed using online services.

I agree to abide by the acceptable usage agreement for school students.

I understand that if I am given a network account and break any of the rules in the agreement it may result in disciplinary action, determined by the principal in accordance with the Department’s Behaviour Management in Schools policy.
