Part 1 - Team Evaluation (your anonymous feedback to your team)

Instructions:

  • Take a step back and honestly evaluate your team's performance. Remember that evaluating is not summarizing or explaining, but rather is judging the quality of the team's performance.
  • Please be honest - your team will not gain or lose points based on whether you identify areas for improvement or strengths – the most important thing is your team’s response to anyissues.
  • Appropriate length varies, but a general expectation is between two sentences and five sentences.
  • Please include coaching suggestions for the team where appropriate.

Part 2- Team member Coaching

Instructions:

  • For the first 5 categories for each team member, provide a specific example of something good you have seen the team member doing that they should continue doing, or a specific coaching suggestion about something the team member should start doing or should stop doing.
  • Please include a mix of positive examples and coaching suggestions. You do not have to include something in every category for every team member, but the expectation is that you will provide input in at least three of the categories for each team member. Your comments will not directly impact the grade of your teammates, so please be honest – the most important thing is the team member’s response to any issues
  • Your positive examples and coaching statements will be graded for their quality (are they specific and action oriented, do they deal with meaningful issues rather than petty items or personality differences, etc.). Appropriate length varies, but a general expectation is 1-3 sentences.
  • For the 6th item, please choose one of the three categories listed. No explanation is needed.
