McKinley Cooperative Checklist

For your child to begin:

1. Application Form with Registration: Must completely fill out one per child, and select which class you want your child to be placed.

2. Health Appraisal Form: (green sheet): This must be SIGNED (not stamped) by your child’s physician. Immunization dates must be complete with month, day and year filled in by the doctor’s office. A child entering the three and four year programs must have had a physical exam no longer than one year prior to his/her entering preschool. A child entering the toddler program must have had a physical exam no longer than three months prior to three months prior to the child entering the preschool. Once you are a current member of McKinley Cooperative Preschool, the health form is good for two years.

3. Birth Certificate: A copy of your child’s birth certificate is required for all new children entering the preschool as verification that he/she is enrolled in the appropriate class.

4. Emergency Card: This must be filled out completely, leaving no spaces blank. Write the word “none” rather than leave a blank space. Enclosed is a sample sheet.

5. Check/Money Order: Registration Fee $35.00, First and last month’s tuition is due prior to your child entering class, made payable to McKinley Cooperative Preschool. Please always write your child’s name, class, type of payment (i.e. Registration/tuition/Material Fee/etc) in the memo of your checks. Please check with the membership chair if you are unsure of the amount due. (Anne-Marie Banko) See reserve side for example. Tuition for the following months will be due by the first Friday of the month and may be turned in at the General Membership Meeting or put in the gray lock box in the recreation room.

*These five items must be turned in prior to your child starting class. You may bring them with you to your child’s first class session.

For you to work in the school:

1. Negative TB Test: We need proof of a negative TB test for you or whomever will be the person working in the classroom on work days. TB test are valid for TWO years. Enclosed is an information sheet on the TB Clinic (OakwoodHospital & MedicalCenter) where you can get the TB test done or you may contact your physician if you prefer.

2. Central Registry Clearance-Background Check: This is required by the Michigan Family Independence Agency (FIA). Please fill out the enclosed request for central registry clearance sheet and drop off at the FIA. The nearest FIA office to the school is on Plymouth just past Inkster. The Office is located north of Plymouth and East of Inkster; there is a closed down Farmer Jack in the same strip mall. They will ask to see your driver’s license. You will be told when you may pick up your clearance.

3. Child Abuse/Neglect Reporting Sheet (enclosed): This is required by state law. Please read the sheets provided and turn in the sheet with your signature.

*These three items must be returned prior to you working in the classroom. You may bring them with you to your first work day.
