Description ofRequested Change/Enhancement: Be specific. Include documentation and additional sheets, if necessary. List general description, fields to be included, requested options, sort orders, etc., as appropriate.
Background: Star measures track important indicators of adherance to high value screening, testing, chronic conditions, medication adherance, and others. UCare is providing client level stars data, and other health plans are interested in providing the same information.
Request:Additional mapping fields in spreadsheet import to accept star rating data. Allow reuse of star data to forms and Word Forms to pre-fill health risk assessment and other documents. Develop client dashboard components to graph results and show areas of improvement for star indicator. Also add Ques/ Reminders for talking points and incentive indicators. Develop summary dashboards for staff caseloads and managers to see across staff and across all clients to determine performance.
Reason: Who will benefit? What is the purpose? What are the efficiencies?
Agencies can improve conversations between staff and clients to encourage adherance to stars measures. Agencies can also track measures across their caseloads and develop interventions to improve outcomes.
