Material Acceptance and Documentation

When out in the field the following may need to be performed for material documentation

-Verify the products on the construction site match the certification that was given to the engineer

-Inspect all manufactured products as soon as possible after delivery

-Refer to the CMM to specify what information is needed off certain materials such as heat numbers, etc.

Certification of Compliance

-For items that required a certification of compliance the following is required;

  • Name of the manufacturer or of the supplier
  • Name and use of the product
  • Statement of specification that the product meets such as AASHTO and/or ASTM and the specification number
  • Signature and title of the person responsible for certifying the product who can bind the company
  • After reviewing the documentation, the project staff member needs to be noted on the certification with his or her initials

Certified Report of Test or Analysis

-The following is required on a certified report of test or analysis

  • Name of the manufacturer or supplier
  • Name and use of the product
  • Statement of specification that the product meets such as AASHTO and/or ASTM and the specification number
  • Lot, batch, heat numbers, etc., applicable to the material delivered
  • Test results for both physical and chemical test requirements as specified
  • Signature and title of the person in responsible charge of certifying the product who can bind the company and the signee job title
  • After reviewing the documentation, the project staff member needs to be noted on the certification with his or her initials

Diary Entries

-Diary Entries should include the following:

  • Description: Brand, model, type, dimensions, lot, heat, etc.
  • Quantity
  • Evaluation and basis for acceptance- visual inspection remarks, product condition, compliance to specifications, etc.
  • Manufacture source and vendor
  • Address should have at least the city and state

-Reference the material diary as an external document to list on the test report index

Test Report Index

Test report index should be requested at the end of the project during finals preparation. The report should be requested from Mike Novey.

Time Schedule

Materials documentation should be done throughout the project. This should not be something that is done at the end of the project. All materials documentation and reporting must be completed and entered in the MTS no more than 60 working days after the work completion date.
