Making it Real Priorities and Action Plan

I have a range of personal centred tools, which I can use as my preferred method of communication (widget).
I actively take part in looking after my home and actively participate in personalising my activities and doing a weekly Activity boardand a nightly staff board. I complete a diary each night and document what I have been doing and if I have enjoyed this.
I take part in ensuring my home is safe by doing the weekly fire checks and health and safety within the home. I have information regarding my care and support in my preferred means of communication. / I have little understanding as to what care is in place for me and I am aware that all my staff are here to support me. However it takes me sometime to process this when my anxieties heighten and I can easily become confused if not communicated clearly to me. / I wish not to partake within my reviews and PCP meeting, as this heightens my anxiety causing me to self-harm. / I am not happy I large crowd however with staff support I am able to overcome this at times. I struggle with changes to my routine however if this is explained to me slowly and within my preferred means of communication then I will accept this. / I am able to express my likes and dislikes. / Concerned I do not know any other individual’sthose are actively part of my care and support. / Providing little information at time, as too much my cause my anxiety to heighten, allow me to decide, how much information I want to have and in my preferred form of communication / Staff that work with me on a daily basis / To discuss this within staff meetings.

I am able to express what I have enjoyed, things I would like to change using my person centred tools in my preferred format. Personalised activity planner, and daily rota.
I am able to access the community using my vehicle and I also walk as a form of exercise to different venues throughout the week. I have been supported by my staff to access the main shopping area and purchase clothes. / My anxieties canheighten when I am around an area that is busy, or if I become unsettled by the activity, over heated or am reluctant to participate. / I have been offered meetings to chat about my local community / In order for my anxiety to not increase, I will be suited in going out at less busy times and within areas that are not busy. I link arms with staff while walking and this helps with balance and makes me feel secure also. / Communication tools and ability to express myself. / I am not able to participate within community fully and meet others from my local area. / Staff is to continue to support and encourage me to actively participate in local activities in which I can participate in. / Staff/ Manager / Staff to discuss ideas within staff meeting, to gain staff input and to look at the risks of increased anxiety. / May 2017

I have also taken an active role in health and safety within my home, completing fire checks weekly and the daily cleaning of my home and completing health and safety checks. I am able to clearly explain using BSL what I like to do and what I feel is suited to me. / I have limited interest in wanting to engage in study or work. I do enjoy activities I have chosen such as word searches.However I am unaware of when events are being held, as too much information or too much knowledge to activity can heighten my anxiety. / I have been given information in advance this only heightens my anxiety as day approaches, I have been given information on the day and that at times can cause my anxiety to escalade especially if I have not been on that activity or a new route. / Learnt that I cannot handle too much information as this can cause me to have heightened anxiety and I cause self-harm. / I am pleased I am able to do more activities out in the community. Iam able to express what my wishes and goals. / I have heightened anxiety, which prevents me from being able to plan WITH my staff and family my goals for the future in the way I would like. As I have to be given little information at a time. / My staff and family continue to provide information, however in small amounts to keep my anxiety levels down. This will allow me not to over think the activity. / Staff/ Managers / Staff and Managers to discuss in team meetings, regular 1-1 with my staff / May 2017
