What: Team trip over Memorial weekend to have fun and compete in Thunder Bay Thunderbolts 40th International Swim Meet.

Open to all Trackers and above.

Indoor 50 meter pool. No minimum times required.

When: Departing at 6:00 am on Friday, May 25, returning about 6:00 pm Monday, May 28.

Where: Meet is held at Canada Games Complex, 420 Winnipeg Ave, Thunder Bay, Ontario, Canada. We will stay at the Victoria Inn, 555 W. Arthur St, Thunder Bay.

How: We plan to travel via Wolfmobile and carpools*.

Swimmers 13 and over may attend without a parent or guardian. 12 and under swimmers must be accompanied by a parent or by a guardian arranged by parent.

*If demand exceeds W-mobile capacity, we may hire a coach bus or add a van.

Cost: Variable depending on package chosen (see attached form).

Saturday Night Sports Buffet (chicken, pasta, salad, bread & juice):

On Saturday night after finals we have a “Sports Buffet” meal arranged at our Hotel for all swimmers on the Meal Plan, and optional for any other swimmers and families. Cost to join is $14 per person (inclusive of tqx and tip).

Sunday Night Team Banquet: Our annual dinner and awards banquet will be held after finals on Sunday night at Kelsey's Restaurant, 805 Memorial Ave. (corner of 8th Ave and Memorial), www.kelseys.ca.

We will have menus and pre-order forms available at the meet, to expedite the service at the restaurant. Checks & gratuity will be settled by individuals with the restaurant at the close of service. The team will provide beverages and dessert.

**Important** Documents needed to cross the border:

·  Athletes ages 18 & under traveling with the team may have a birth certificate OR a passport, plus written parental consent (satisfied by the attached Authorization form).

·  Children ages 15 & under traveling with family may have a birth certificate OR a Passport.

·  Children ages 15 & under traveling with a non-parent guardian additionally require verified written consent of both parents.

·  Children ages 16-18 NOT traveling with the team need a Passport.

·  All US citizens Age 19 and older need a Passport

Honor Code of Conduct

The following code is in effect throughout the year. Some of the items refer specifically to team travel. Additionally, anyone who, in the opinion of the coach or coaches, acts in a manner that would interfere with the travel objectives listed below, will be subject to immediate return home (at the expense of the parent and/or swimmer) and other punishments including barring from future travel meets or other competition or dismissal from the team.

GWST may travel as a team to meets and swimmers are expected to behave in an exemplary manner. The reputation of GWST, as well as the other athletes with you, is dependant on your behavior.

The coaching staff holds the final word on any rules, regulations, or disciplinary action.

The consumption or purchase of alcohol, purchase or use of Fireworks, smoking or chewing tobacco, or use of any other illegal drug or substance of any kind will not be allowed, In addition, any team member found or suspected to be in the presence of others (regardless of team affiliation) partaking in any of the above activities will be subject to the same punishments and probable expulsion from GWST.

At no time will male and female athletes be in the same room together with the door closed. This applies to GWST members or members of any other team.

No team meetings may be missed. Be punctual to all meetings and warm-up times.

No team member may be out of their room after the assigned bed time. Permission must be obtained for the coach to leave the room past this time.

Any damages or thievery incurred at a motel or dorm will be at the expense of the swimmers assigned to that room, and further disciplinary action will be taken. No loud or boisterous behavior will be tolerated in the always or public areas, and such behavior should be kept to a minimum in your rooms. Make all long distance calls on a credit card or collect.

All team members will be polite in restaurants and leave a tip. If there has been a problem with the service, see the coach.

Violations of this Honor Code of Conduct may result in a phone call home to inform the parents that the swimmer is being sent home on the next available flight. Expenses for this will be the responsibility of the parent or guardian of the swimmer.

I recognize my responsibility to abide by the rules and requirements of Great Wolf Swim Team that I am representing and I acknowledge that I have received and read.
