The student may not participate in Physical Education until this form is returned.
Physical Education Procedures
1. Purchase the uniform from your instructor.
2. Your name must be clearly written and visible on your uniform.
3. For health reasons, white socks are required.
4. In cold weather, any sweat clothes are acceptable.
5. If a class is missed for a medical reason, or the student is absent from participation, library work will be required for grading purposes.
6. It is the student’s responsibility to contact the teacher for make up work.
7. Keep your combination a secret. Use your assigned locker and keep your valuables locked.
8. Jewelry and gum are not allowed.
9. Athletic shoes must be worn.
10. A student is not allowed in the gym area without a teacher present.
11. Long hair must be tied back.
12. The uniform cannot consist of any shorts hanging out below the uniform shorts.
13. Respect others.
14. Keep your hands to yourself, and on your own possessions.
15. Each student is responsible for his or her own behavior.
16. Do not cut through any other classes while they are in session, or on the fields.
17. In order to use any physical education equipment, a teacher must give you permission and be present. 18. Follow all safety procedures.
LOCKER ROOM RULES
1. Lock up all of your valuables during class.
2. Be polite to others.
3. No loud voices.
4. Lost and found items are on the table.
5. Do not share your combination with anyone.
6. Respect everyone’s privacy.
7. Change quickly and go to your class.
8. Put garbage into the trash baskets.
9. Do not take anything off the bulletin boards.
10. Keep the locker room clean.
11. Keep your hands off of other’s belongings.
