Under limited supervision serves as a coordinator of services for a program area such as HANDS, Home health, School Health, Health Start, etc. Manages program resources. Assures that accurate information is documented appropriately according to program standards. Prepares programmatic reports as required by the local, state or federal guidelines. Participates in Bioterrorism and Emergency Preparedness activities.
Makes observations of the delivery and quality of client care and leads quality improvement activities; reviews schedules for assigned worksites; prepares work schedules and makes daily nursing assignments including any follow-up activities; participates in meeting of supervisors and other administrative staff and models effective clinical decision making skills. Can fill in as county supervisor in absence of such.

  1. Coordinate total quality management program and activities that reflect the home health care agency’s philosophy and standards of home health and nursing care of clients.
  2. Initiate Adverse Event Outcome Report investigation and subsequent plans of correction.
  3. Coordinate Outcome Based Quality Improvement activities and implement recommendations to ensure that desired outcomes are achieved and maintained.
  4. Evaluate all patient satisfaction surveys and complaints and investigates accordingly.
  5. Evaluate all physician satisfactions surveys and complaints and investigate accord
  6. Develop, implement and evaluate quality management program and activities that support the home health care agency’s objectives in delivering quality and safe home health care services and meets Conditions of Participation.

  8. Serve as a resource person on clinical, professional and reimbursement issues.
  9. Compile and summarize data for presentation at monthly departmental meetings and quarterly meetings as requested.

  1. Conduct comprehensive autonomous and/or collaborative health assessments to determine the health status, functional and psychological need and competence of patients and their families within the context of their environment and social supports.

  3. Establish and comply to visit frequency according to the patient’s need and physician orders.
  4. Analyze information by using critical thinking skills and creative problem-solving analysis to determine appropriate nursing actions, implications, and limitations.
  5. Demonstrate the ability to perform a wide range of nursing procedures independently including but not limited to wound care, IV therapy, injections and catheterizations.

  7. Assess the safety of the home environment with the goal of optimizing patient safety and taking actions to support a safe work environment for all members of the team.

  10. Understand the reporting procedures for abuse, neglect and exploitation.
