This completed form must accompany each nomination. Please also prepare and submit a bio on the nominee that does not exceed two pages. In the bio, please include all significant accomplishments and details that qualify the nominee for this award. Submissions that do not include this completed form and a maximum two-page bio will not be considered. Although not required, you may also include a CV or resume, letters of recommendation, and any other relevant supporting documents. All information is subject to editing at the discretion of the selection committee.

The Livingston Alumni Association (LAA) established its Distinguished Alumni Award in September 2000 to recognize Livingston College alumni who have distinguished themselves by contributions they have made in their chosen fields of endeavor, by the leadership they have exhibited, and by the general benefits to the larger society resulting from their activities. The Livingston Legacy Award was established in 2009 to recognize Livingston College faculty and staff that played a key role in the establishment and growth of Livingston College and its mission, and contributions to the overall Rutgers and global communities.For bios of previous honorees, see
Historically, the LAA awarded the Seth Dvorin Distinguished Young Alumni Award in 2004, 2006 and 2009, to Livingston College distinguished young alumni 30 years old and younger. The award was established in memory of Army First Lt. Seth Jeremy Dvorin (LC'02, Administration of Justice), who was killed in Iraq on February 3, 2004.
The honorees will be celebrated at an awards dinner, tentatively scheduled for the fall semester of 2017, at or near the Rutgers-New Brunswick/Piscataway campus.
