b.  Identify who can assist you in achieving health goals outside your family
Specific Learning Objective(s) / The student will identify actions needed to achieve a person’s health goal by identifying healthy and unhealthy food for teeth.
Stage 2 Assessment Evidence
Assessment Instrument
(Ex. Test, Poster, Presentation, Picture, etc.) / Students will be given a worksheet and a grocery coupon whereby they will identify which foods are healthy and unhealthy.
Evaluation (Grading) Instrument
(Ex. Point System, Rubric, Checklist, etc.) / Students will cut out and sort foods that are good for teeth and bad for teeth. A Checklist will be used to verify if the students sorted the foods correctly.

Discussion about the tooth fairy.
What is a tooth fairy? Has the tooth fairy been to your house yet?
Teacher will discuss how important it is to take care of your teeth.
Teacher will pass out the worksheet on good and bad foods for teeth.
Teacher will also pass out a grocery coupon for each student.
Teacher will explain that the students need to cut and sort out the foods that good and bad for teeth. After the students cut and sort they will glue the food on the correct side of the worksheet.

Students will discuss What is a tooth fairy? Has the tooth fairy been to their house yet?
Students will cut and sort out foods that are good and bad for teeth.
Students will glue the foods on the correct side of the worksheet after they are done cutting and sorting the different foods.
Teacher’s Lesson Closure/Wrap-Up/Transition / Students will volunteer to show their worksheet when completed.
Adaptations for Exceptional Students (Anyone who requires modifications for their needs) / Students who need help sorting the good and bad foods will be allowed to work with a partner.
Related Activities/Extensions
(What can students do who need more than is in the lesson? Should be related to lesson.) / Students will write a story about telling how teeth are important. In the story the student needs to explain ways to keep teeth healthy. The student can also include the tooth fairy in their story.
