Video clip 1. Anteroposterior fluoroscopy showing engagement of a 6 Fr XB 3.0 SH guiding catheter (Cordis Corporation, Miami, FL, USA) into the ostium of the left coronary artery followed by advancing a Runthrough extrafloppy guide wire (Terumo Co., Tokyo, Japan) into the left main artery.

Video clip 2. RAO cranial fluoroscopy showing the first guide wire in the first diagonal branch. To reduce the risk of wire perforation, the Runthrough guide wire had been had replaced by a Neo’s Rinato guide wire (Asahi Intech, Japan) via Finecross microcatheter (Terumo Co., Tokyo, Japan). Consequently a second guide wire (Runthrough extrafloppy) is placed into the left anterior descending artery with support of a Finecross microcatheter.

Video clip 3. RAO caudal fluoroscopy showing implantation of a Xience V 3.0/28 mm drug eluting stent (Abbott Vascular, Abbott Park, IL, USA) into the midproximal left anterior descending artery. Stent prolongation using a Xience V 2.5/15 mm drug eluting stent is performed in RAO cranial projection.

Video clip 4. The initial IVUS pullback sequence of the left anterior descending artery begins just at the distal end of the lesion, corresponding to a position slightly proximal of the designated distal stent landing zone (B5 as indicated in Figure 2). Position B4 in Figure 2 corresponds to frame 667, B3 to frame 905, B2 to frame 1021 and B1 to frame 1233. The frame number is indicated in the right upper corner. The motorized pullback speed was 1 mm/s and the frame rate was 30 Hz.

Video clip 5. The final IVUS pullback sequence starts in the distal left anterior descending artery, shows the distal stent edge in frame 492 and the proximal stent edge in frame 1799 at the ostium of the left circumflex artery. The stents are fully expanded without significant malapposition and no dissection, hematoma or severe flow impairment can be detected. The ostium of the first diagonal branch is nicely preserved (frame 1411). The frame number is indicated in the right upper corner. The motorized pullback speed was 1 mm/s and the frame rate was 30 Hz.
