Directions: Your child has been a participant in the Title I School-Wide Program during this school year. The purpose of this questionnaire is to gather information on how well the school communicates with parents of children who receive Title I services. Please take a few minutes to complete the questionnaire. Your feedback will be used to improve parent involvement activities. Suggestions on how we can better help our students at home or at school are welcome. We will summarize the results from this questionnaire and use the information to improve school programs and parent involvement offerings. For more information, please contact the Title I coordinator, Teresa Alsobrook.

Please check YES or NO for each question. To thank you for your time, please return a paper to LCMS with your child indicating that you completed the Title I survey (on-line or hard copy; if hard copy, this completed survey must accompany note), include the date that you completed it, put your child's name on the paper, and then you need to sign it. If you return a signed paper by April 21 indicating you completed the survey, your child will receive a HW pass that expires May 9. Thank you for your valuable input.

This information is in the LCMS Student Agenda (pg 21-24; you signed and returned pg 23-24) and was discussed at Open House on August 2, 2013, and PTSO on Aug. 12, 2013.

  1. Did you receive a copy of the Parent Involvement Policy and the HomeSchool Learning Compact?

This information is in the LCMS Student Agenda (pages 20-24; you signed and returned The Compact from pgs 23-24) and was in packet at Open House on August 2, 2013, and discussed at PTSO on Aug. 12, 2013. This information was also provided at the Back To School Fair on July 29, Home Visits on August 2, etc

Invitations were mailed home to all students on June 10, info is on the cover page of the agenda/IC parent portal/website, invitations are in the back of the District Summer Catalog, an invitation is in the agenda on page 18, one calls/text/emails were sent for district-wide conferences on October 21, during School Day, before/after school, etc.

This was in the invitation mailed home on June 10, in the back of the summer catalog, and on page 18 in the LCMS Student Agenda. This was done at the August 12, 2013, PTSO meeting.

Each quarter, you should receive progress reports every 4.5 weeks and report cards every 9 weeks. These dates are published in the agenda on pg 11, IC portal, website, marquee, social media, one calls, announcements, newsletters, etc.

If you will look at the policy on the bottom of page 20 in the LCMS Student Agenda, it asks for input on August 12 or by contacting the principal. Parents were invited to this meeting through mailed letters on June 10, agenda page 18, Summer Catalog, BTS Fair, Open House, Home Visits, policy, etc. This was done at the August 12 PTSO meeting.
