1. Do you currently have cash/unrestricted operating reserves? If so, note how many months of reserves you have and how you plan to grow the amount in the future.
2. If you do not have an operating reserve, what is your plan for creating and growing one?
3. Endowment Fund: Note if your organization has an endowment fund, and if so, include approximate endowment fund value and asset custodian.

- Briefly describe your organization’s mission, the constituency you serve and the programs/services you offer to the community.

- Project Description: Briefly describe the project/purpose you are seeking funding for and mention:
- The issues that will be addressed, the significance of those issues, and their impact on McLennan County. Be sure to include a statement about the need for this program/service as well as data that indicates why this project is needed at this point in time.
- The populations that will benefit and estimate the number of people/units to be served (include demographical information). Indicate the proposed cost per person/unit to be served.
- Describe your action plan/strategy and how your project will expand or complement existing community services.
- Identify any similar service providers as well as any arrangements to collaborate with other organizations if appropriate. If your organization delivers services within the area of health, education and/or financial security, be sure include information on how your organization is collaborating with Prosper Waco and which of their goals your organization is aligning itself with.

List the anticipated outputs andoutcomes for the project.

Describe how the project furthers the mission and priorities of Waco Foundation.
- Project Budget
- Budget Determination:
Describe how the cost of the project was determined (e.g., competitive bids, architect, and contractor). State the total capital budget and project’s annual operating costs. Explain your plan for financing maintenance/operations after the facility is completed/renovated or equipment has been purchased. Give your capital campaign goal and indicate how you determined its feasibility.
