This form requests your permission to allow your child to travel to all meets during the 2016-2017 school year, for the purpose of competing at UIL sanctioned tournaments. These competitions will sometimes require that your child travel to schools within our district, the Rio Grande Valley, the Region and beyond. All students must travel to and from competition in a school designated vehicle, unless a written agreement is presented to the UIL Department before initiating the travel. The school will ensure that the students be supervised during these trips and that all precautions will be taken in the interest and well being of each student: however, we cannot assure you that accidents will not happen.
Students are subject to all school rules and policies and are required to adhere to the travel procedure of the UIL department. We agree that sponsors and La Joya I.S.D. cannot be held responsible for any accident or misfortune which might occur in connection with these trips.
Included in this form is an authorization for your child to be treated and/or hospitalized by a physician in case of an accident or illness. If your child has any special medical needs or is allergic to any medication or treatment please list those below.

Esta forma solicita de su permiso para permitir que su hijo/hija viaje y compita en los torneos sancionados de UIL del año escolar 2016 – 2017. Estas competencias requieren que a veces su hijo/hija viaje dentro del distrito, el valle de Rio Grande o la región. Todos los estudiante deben de viajar dentro de un vehículo asignado por el distrito al menos de que haya un consentimiento por escrito presentado ante el departamento de UIL antes de iniciar un viaje. El personal de cada escuela se asegurara de supervisar a los estudiantes durante dichos viajes. Se tomaran todas las precauciones mediantes para asegurar el bienestar de su hijo/hija. Sin embargo, no podemos asegurar de que no habrá accidentes. Todos los estudiantes están sujetos a seguir las reglas, pólizas y procedimientos de departamento de UIL. Concordamos que los patrocinadores y el distrito de La Joya no se hará responsable por algún accidente o desgracia en caso de que ocurriera en estos viajes.
Esta forma incluye su autorización para que su hijo/hija sea tratado u hospitalizado en debido caso de un accidente o enfermedad. Si su hijo/hija tiene alguna alergia o caso medico en especial, por favor de mencionar en la línea posterior.
