JOB PURPOSE

As part of the multidisciplinary team (MDT), the post holder will provide of direct patient care across Kilbryde Hospice. As a senior member of multi-disciplinary team (MDT) the post-holder will carry out duties as required to contribute to effective communication and provision of care either in the hospice or in the patient’s home. The post holder will at all times work with care, compassion, respect, dignity and commitment.

ENVIRONMENT

Kilbryde Hospice provides specialist palliative care services to the people of South Lanarkshire.It is a specialist resource for those individuals receiving palliative care who have particularly complex needs. The Hospice strives to meet the physical, emotional, social and spiritual needs of patients and carers, offering support in coping with the practical implications of life-limiting illnesses. Staff are required to provide the highest standard of patient-centred care possible, while promoting choice, dignity, self-esteem and an enhanced quality of life. The Hospice is a Company Limited by guarantee, and a registered charity, and is required by separate and differing legislation to meet finance, governance and regulatory requirements.

KEY ACCOUNTABILITIES

To provide patient centred and evidence based specialist palliative care to patients and carers.

Work as a senior member of the multi-disciplinary team promoting a MDT approach to care.

Actively promote a rehabilitative model of care.

To act in accordance with the Nursing Midwifery Council (NMC) code of professional conduct and Kilbryde Hospice policies and procedures.

Contribute to education and research.

Clinical

Demonstrate competence of the specialist palliative care environment recognising the needs of Kilbryde Hospice patients, carers and relatives.

Assess, plan, implement and evaluate the care of patient and carer.

Administer medication and treatment as prescribed in accordance with NMC and Kilbryde Hospice’s medicines policies and procedures.

Conduct holistic assessment using the OACC suite of measures.

Act as a patient/carer advocate and always in the best interest of patients and their carers.

Demonstrate competency in your knowledge and clinical skills.

Demonstrate a current knowledge of infection prevention and control practices.

Evidence competence in moving and handling.

Demonstrate competence in admission, transfer and discharge of patients from Kilbryde Hospice.

Communication

Demonstrate knowledge of effective person-centred communication skills and barriers to communication.

Demonstrate knowledge of effective communication and why this is integral to the delivery of palliative and end of life care.

Maintain accurate and contemporaneous written and electronic documentation.

Liaise and communicate with external services e.g. GP, District Nurses, community palliative care team, neighbouring hospices.

Maintain confidentiality in keeping with Kilbryde Hospice Confidentiality Policy and Caldicott Guidelines.

Information Technology

Demonstrate competence with Microsoft Office, Excel, Crosscare.

Ability to record confidential patient data using Crosscare or similar.

Have an understanding of Kilbryde Hospice Information Governance policies.

Management

Contribute to the management and leadership of Kilbryde Hospice clinical services.

Take responsibility for the management role of nurse in charge when appropriate.

Actively participate in clinical effectiveness and audit to enhance service development.

Ensure resources are adequately sustained by maintaining appropriate levels of supplies and stocks.

Education & Research

Participate in the education, training and supervision of staff and volunteers and health and social care partners.

Demonstrate an awareness and a willingness to engage in NHS Education for Scotland framework for palliative and end of life care (2017).

Ensure practice is updated in line with Revalidation and NMC requirements.

Demonstrate a commitment to ongoing learning and development and to participate in any training relevant to the role.

Participate in reflection and, if appropriate, assist in the clinical supervision of other staff.

Participate in Kilbryde Hospice Personal Development Review and competency framework.

Actively contribute to audit and research.

KNOWLEDGE, TECHNICAL SKILLS, QUALIFICATIONS AND EXPERIENCE

Essential:

Registered Nurse with post registration experience in palliative care.

Relevant Nursing Degree.

Evidence of Continuous Professional Development (CPD).

Ability to practice as a safe competent practitioner, applying evidence based practice.

Excellent communication skills – both written and verbal.

Good knowledge of professional accountability and responsibility and the limitations of the role.

Ability to identify problems and act appropriately.

Ability to manage a team and caseload.

Ability to deal with difficult/sensitive situations.

Ability to manually handle patients andoperate lifting aids.

Able to use information technology to input into electronic patient records and/or skills to participate in on-line learning programmes.

Experience of working with minimal/without supervision.

Desirable:

Understanding of clinical governance and its requirements.

Relevant qualification in speciality.

Teaching qualification.

Experience of supervising people and managing resources.

Have a good understanding and comply with relevant Kilbryde Hospice Policies and Procedures at all times.

The Data Protection Act 1974/1998 and GDPR (2018) safeguards the handling of information held in both electronic and manual filing systems and it is the duty of all staff employed by Kilbryde Hospice to uphold the principles of the Act.

All employees of Kilbryde Hospice must be aware of infection prevention and control policies and are expected to follow them at all times. Any breach of infection control policies which places patients, visitors or colleagues at risk may result in disciplinary action.

The post holder is required to familiarise him/herself with and comply with the Kilbryde Hospice Health & Safety policies.

The post holder must at all times carry out duties and responsibilities with regard to the Kilbryde Hospice Code of Conduct.

Kilbryde Hospice operates a strict non-smoking policy in the hospice or Hairmyres hospital grounds.

The post-holder must at all times carry out his/her responsibilities in line with Kilbryde Hospice Dignity at Work and Equal Opportunities Policy.
