Patient and Family Advisory Council Charter

Vision

Our Patient and Family Advisory Council (PFAC) partners patients and their family members with health care providers to enhance experiences at Kaweah Delta. Patients and their family members serve on our PFAC to ensure the patient and familyperspective isused to co-design safe, high-quality patient-centered care and services.

Membership Guidelines(To be finalized through co-designwith Patient Family Advisors once council is established)

  1. Patients, family members, and health care providers from Kaweah Delta are eligible to participate. Members must be committed to building a partnership of advisors and employees working to understand the needs of the community they represent. They must support collaboration to co-design programs and policies to effectively address health care challenges and enhance patient experience.
  2. The group will consist of 6-12 advisors (patients, caregivers, and/or family members)to allow for open andvibrant discussions. The PFAC will also include 4-6 Kaweah Delta employees.
  3. Membership term is 2 years. Members whose terms are expiring may re-applying for membership.
  4. All members will participate in volunteer orientation. Orientation will include the PFAC’s vision, membership guidelines, and structure.
  5. All members of the PFAC will sign a confidentiality agreement.
  6. Recruitment for future members will be ongoing. Interested parties can submit an application via our Kaweah Delta website. Final candidates for available positions will be interviewedand selections will be made.

Structure(To be finalized throughco-design with Patient Family Advisors once council is established)

  1. The council will meet in-person monthly for 90-minutes.Food will be provided.
  2. The PFAC will be facilitated by two co-chairpersons (one member from the community and one member from Kaweah Delta). Co-chairs will develop the agendas and keep meeting minutes. (Plan is to have co-chairs in place by month 4)
  3. Agenda items can be added by contacting a co-chairperson prior to the meeting. Who, what, and when will be captured in the minutes for every agenda item that requires follow up.
  4. Members are expected to participate in a minimum of 6 PFAC meetings a year. Advisors will select additional project(s) or committee(s) based on their time, interests, and availability.
  5. A PFAC Annual Report will be created to publicize the council’s accomplishments from the preceding year. This report will be provided to the Board of Directors, medical staff, and the community.
