2018 Grant Solicitation Categories
Kaiser Permanente is seeking community-based service providers to implement specific strategies that directly link with our plan to address health needs in this community. Funds must be used to serve vulnerable, low income populations of this Kaiser Permanente service area, which is northern San Mateo County.
Each organization may submit a Letter of Intent (LOI) for only one of these grant categories and strategies. Applicants will be evaluated based on fidelity to the stated goals, expected outcomes, and targeted populations.
- Healthy Eating / Active Living (HEAL)
Long-Term Goal: All community members eat better and move more as part of daily life in order to prevent and reduce the impact of chronic conditions (e.g. obesity, diabetes, CVD).
Intermediate Goal: Improve healthy eating and physical activity among residents in low income, under-resourced communities.
Target Populations: Older adults, African Americans, Latinos esp. youth, Filipinos and Pacific Islanders.
Strategy:Increase access to healthy, affordable foods, including fresh produce, and decrease access to unhealthy food.
Expected Outcome: Increased consumption of fruits and vegetables.
Strategy: Reduce access to and appeal of sugar-sweetened beverages.
Expected Outcome: Decreased consumption of sugar-sweetened beverages.
Strategy: Increase access to physical activity opportunities in the community.
Expected Outcome: Increased physical activity.
Strategy: Increase access to safe parks and public spaces. community.
Expected Outcome: Increased use of parks and public spaces.
- Access to Care and Coverage
Long-Term Goal: All community members have access to high quality, culturally and linguistically appropriate health care services in coordinated delivery systems.
Intermediate Goal: Increase access to comprehensive health care services for low income and vulnerable populations.
Target Populations: Uninsured adults, Older adults, Men, African Americans, Latinos, Asian/Pacific Islanders.
Strategy:Increase and systematize access to needed social non-medical services.
Expected Outcome: Increase in referrals and coordination to non-medical social services.
- Behavioral Health
Long-Term Goal: All community members experience social-emotional health and wellbeing and have access to high quality behavioral health care services when needed.
Intermediate Goal: Increase access to culturally and linguistically appropriate behavioral health services for vulnerable and low income populations.
Target Populations: Children & teens,Young adult males (age 18-24), Middle-aged adults (40-64), Older adults, Women, African Americans, Latinos, Asian/Pacific Islanders, Caucasians.
Strategy:Provide access to programs, services or environments that evidence suggests improves overall social/emotional wellness.
Expected Outcome: Increased enrollment in programs to improve social/emotional wellness.
Strategy:Improve navigation to appropriate behavioral health care within the healthcare system and support services in the community.
Expected Outcome: Increase in numbers of low-income patients who receive behavioral health care services.
