OVERVIEW
The County of Fresno on behalf of the Department of Community Health is seeking proposals from qualified health care providers for the provision of comprehensive health care services for juvenile detainees held and housed at the County of Fresno Juvenile Justice Campus. To date, health care services for juvenile detainees and service coordination has been provided by the Department of Community Health. It is the County’s intent to contract these services with the opening of the new Juvenile Justice Campus, slated to populate in June, 2006. Mental health and substance abuse counseling services are currently, and will continue to be, provided by the County of Fresno Department of Children and Family Services (DCFS). Therefore, the successful vendor will be expected to work in collaboration with DCFS, in addition to other County Departments and personnel.
The County intends to award a five (5) year contract. It is expected that the resulting contract between the successful vendor and County will provide for the following:
- Delivery of quality, cost-effective, health care services in full accordance with standards established by the California Medical Association (CMA), the California Administrative Code (CAC), and the laws of the State of California;
- Provision of effective management services of health care professionals;
- Development and implementation of a health care service program with clear objectives, policies, and procedures;
- Maintenance of complete and accurate records of juveniles’ care and collection and analysis of health care statistics on a regular basis;
- Utilization of appropriate health service personnel, in accordance with their scope of practice, who are certified and licensed as required in California;
- Administrative leadership that provides for cost accountability, report delivery, and responsiveness to the Contract Administrator’s requests;
- Maintenance of an open and cooperative relationship with County of Fresno staff;
- Continuing education for health care staff; and
- A health care system that is operated in such a way that is respectful of the detainee’s rights to basic health care.
The successful vendor must provide a program that meets the general and unique needs of the juvenile detainees and inclusive of the full scope of services, as specified herein. In addition, must demonstrate an ability and a commitment to provide and/or conform with all health care services operation activities, and administrative/management practices in compliance with CAC Title 15 guidelines, California Penal and Health & Safety Codes, and all other applicable laws, codes, and regulations.
The County estimates a total annual budget of $1,401,512, which includes Salaries and Benefits at $1,288,549 and Services and Supplies at $112,963 for fiscal year 2006-07, if services were to continue to be providing by the County Department of Community Health.

Description of Juvenile Facility and Population
The Fresno County Juvenile Justice Campus (JJC), currently under construction, is a state of the art facility located on 220 acres on the southwest corner of American Avenue and Freeway 99 approximately 7 minutes from downtown Fresno. The first of a four phase construction project includes a 480 bed facility scheduled for occupancy in June, 2006 at an estimated population level of 330. The juvenile population is estimated to increase as follows: 360 in January, 2007; 420 in January, 2008; and reach the rated capacity of 480 in January, 2009.
The JJC is divided between detention and commitment programs, and will include an InstitutionalCoreBuilding consisting of intake, medical and mental health facilities, as well as institution administration. The CoreBuilding includes two furnished examination rooms with locked medical storage and a medical staff workstation, locked medical and pharmaceutical storage, locked records storage, medical staff workstations, medical administration offices, and a medical staff locker room. The Detention and Commitment Housing Areas include a total of five furnished examination rooms.
Average Length of Stay
The average length of stay at Juvenile Hall in fiscal year 2003-04 was 43.2 days and 41.4 days in fiscal year 2004-05.
There were 4,353 admissions in fiscal year 2003-04 and 4,484 in fiscal year 2004-05.
Medical Needs
Currently, each ward receives an initial medical screening at booking for communicable diseases, other illnesses, injuries, pregnancy, drug and alcohol use, and mental health problems by Probation staff. Within 96 hours after admission each minor also receives a complete physical examination including appropriate laboratory tests and Tuberculosis screening.
The medical care that is provided is essentially that provided by "urgent care" or "walk-in" clinics. This includes treatment of routine ailments and injuries, X-ray services, laboratory services, passing medications, nursing treatments, including time-sensitive treatments, for example for diabetics, and patient education. Additionally, wards may request a referral to medical staff at any time. Staff also initiates referrals to medical staff if they observe signs or symptoms indicating a medical condition.
Patients are referred to University Medical Centers for treatment of conditions that require specialized medical expertise beyond that available from a general practitioner or pediatrician, emergencies, surgery, hospitalization, and urgent and/or painful dental conditions.
