JOB PURPOSE: To provide quality services via support to the case manager on duty and the young women engaged in the program that are homeless through case management, education and mediation. To resolve crisis and promote empowerment to these young women to make healthy choices to improved quality of life and towards stable and secure housing and to ensure that an acceptable standard of household safety, security and cleanliness is maintained.

KEY RELATIONSHIPS

General Manager / Follow Policy and Procedures, receive direction and guidance and ask questions
Team Leader / Follow Policy and Procedures, receive direction and guidance and ask questions
Colleagues / Follow Grievance Procedures, treat colleagues professionally and with respect. Provide feedback, clear guidance, hand-over and provide assistance and support to colleagues as required.
Client / Follow Code of Conduct, Mandatory Reporting, confidentiality, case management guidelines

DUTIES AND RESPONSIBILITIES:

Case Management and Mediation

  • Undertake orientation of clients and monitor their progress
  • Inform Clients of their rights and provide knowledge and skills to enable clients to access other services as required
  • Identify the client’s presenting problems and underlying areas of concern, determine appropriate levels of support
  • Encourage clients to attend school, training programs, work or other constructive activities
  • Organise and participate in recreational activities
  • Provide support to empower and enhance the functioning of the presenting client in line with program guidelines
  • Mediate with the client to resolve conflict and negotiate agreed goals and actions
  • Provide educative information to the general public seeking information as appropriate
  • Respond appropriately to clients in crisis situations
  • Update progress notes
  • Provide telephone support to clients where appropriate
  • Support clients to reconnect with their family or community (where appropriate)
  • Comply with Policies and Procedures
  • Inform, guide and enforce OH&S Standards ensuring a safe environment for clients and visitors on site

Administration and Supervision

  • Fill in incident and injury forms
  • Advocate on behalf of clients
  • Answer phone and reply to messages
  • Undertake other administrative and office management tasks
  • Adherence to TGREEO and OHS Standards
  • Supervise and assist clients with chores, organise and participate in cooking and general living skills development

Teamwork and Agency Development

  • Undertake additional appropriate tasks as required
  • Actively contribute to Quality Improvement Committee activities, staff appraisals and training sessions
  • Contribute to the development, implementation and evaluation of the agency service model and service delivery plan, policies and procedures
  • Participate with peers in facilitated staff group support/supervision and debriefing sessions as required
  • Participate in agency planning workshops as required
  • Maintain appropriate levels of skills and knowledge of youth and homeless issues and trends through regular training
  • Develop and maintain a working knowledge of other agencies and services for appropriate referral of clients
