HOURS OF WORK: 28 per week

SALARY:£16,710 actual, £20,880 pro rata FTE

1. JOB PURPOSE:

1.1To engage with vulnerable people in the community who are socially isolated following a life changing experience such as bereavement or a fall. This includes supporting individuals with a long term medical condition who reside within their home to prevent unscheduled acute hospital admissions; by ensuring support is in place to enable them to effectively manage their condition/situation.

1.2To work in `Partnership with colleagues’, working across the continuum of care provided by the Charity and beyond so as to complement other community services.

1.3To recruit, train and support suitable volunteers for the project.

1.4To work within the model of outputs and outcomes; developmental targets will be identified and implemented, regular progress reports to be prepared and submitted.

2. PRINCIPLE DUTIES AND RESPONSIBILITIES:

2:1To be proactive in developing and maintaining close working relationships with potential referrers across the city, including G.P’s, health and social work professionals, and other voluntary sector agencies.

2:2 To position the service so as to complement other community services including those provided by Age UK Salford; to develop an awareness of the wide range of social activities, groups and events available in neighbourhoods and may be usefully enjoyed by individuals; to facilitate the sharing of this awareness with volunteers’.

2:3 To work in Partnership with colleagues’, working across the continuum of care provided by the Charity and beyond. To work within the referral, recording and monitoring systems in order to efficiently operate, monitor and evaluate the service. To collect and collate data and compile reports required by Age UK Salford and/or funders’.

2:4 To be innovative and proactive in recruiting and training suitable volunteers’ into the project in line with Age UK Salford’s policies and procedures.

2:5 To provide regular support, monitoring and contact to volunteers in partnership with the volunteer coordinator.

2:6 To arrange the appropriate induction and ongoing training to volunteers’ and to evaluate its effectivenessin partnership with the volunteer coordinator.

2:7.To receive and manage referrals’ to the service and to visit potential clients’ in order to establish the suitability of the service for them; to raise clients’ awareness of possibilities and to help identify their individual goals.

2:8To match suitable volunteers’ to clients and to undertake an introductory visit with the potential volunteer.

2:9To allocate & monitor casework to volunteers’ systematically.

2:10To assist in the development & application of suitable models of intervention in order to achieve desired outcomes.

2.11To be proactive in developing suitable publicity and information materials alongside the Marketing Manager in order to effectively promote the service and to encourage referrals and volunteer applications.

2:12To undertake regular formal and informal evaluations of the service and to include such evaluations in internal and external reports.

2:13To be proactive in networking effectively in the community in order to raise awareness of the service and the work of Age UK Salford in general. To utilise a range of innovative mechanisms to aid promotion and communication to ensure that all Age UK Salford’s policies and procedures are adhered to, and that services are provided in a confidential and equitable manner.

2:14To attend regular formal supervision with the line manager and to undertake such training deemed necessary for the fulfilment of the role.

3. ADDITIONAL RESPONSIBILITIES:

3:1 To ensure compliance with all relevant service standards, policies and procedures of

Age UK Salford (ACS).

3:2.To developand maintain effective relationships with service users, their carers and all other key individuals and / or groups whilst demonstrating a positive commitment to providing good customer service at all times.

3:3. To communicate effectively by maintaining clear, concise and open communication at all times, ensuring that the Service Manager is kept fully informed and updated.

3:4.To attend and make a positive contribution to regular team meetings and individual supervision.

3:5.To continuously review own performance and take responsibility for own learning and development by undertaking induction and on-going training as required.

3:6.To work collaboratively with the other team members, external professionals and other key stakeholders to consistently deliver positive outcomes for those who use our services.

3:7.Demonstrate personal leadership by contributing toward continuous service improvement, adopting a positive approach to change and willingness to seek solutions to resolve the day to day challenges of delivering effective services for older people.

3:8.To ensure that the service offered is sensitive and responsivein meetingthe needs of the diverse communities in which they are delivered.

3:9.To undertake any other tasks, duties or projects as required which are relevant to the role.

3:10. To support promotional events, fundraising and other income generation events and any other duties a may seem reasonably required by the organisation from time to time within the context of the job description.

PERSON SPECIIFICATION – Social Rehabilitation Co-ordinator

The following requirements will be assessed from a combination of information provided on the application form, panel interview process and references:

Education and Qualifications / Essential / Desirable
Relevant NVQ level 2 or equivalentand willingness to work toward level 3 / X
Skills, Knowledge and Experience / Essential / Desirable
At minimum of two years experience of managing people (paid or unpaid).
Experience of working with older people in a paid or unpaid capacity; you will have empathy, with an insight into older age and an understanding of the impact of social isolation.
Understanding of the principles of safeguarding vulnerable adults’.
Awareness and understanding of relevant social policy and legislation.
Understanding of the identification, assessment, management and review of risk in relation to clients’ referred to the service.
You will have excellent interpersonal and communication skills, work in a person centred way with people and interested in finding out about their lives and wishes and have enthusiasm for working with people from diverse backgroundsand different communities’.
Experience of completing monitoring and evaluation tasks and/or establishing accurate record keeping systems.
Excellent written and verbal communication skills including basic IT skills.
Understanding of the principles of confidentiality and equal opportunities.
Ability to deliver training programmes’.
Experience of undertaking an initial assessment of clients’ needs, in order to offer choice, variety and flexibility and provide a person centred service.
Experience of project work or action planning, including implementation and review.
Experience of working with volunteers’.
Experience of developing community services’ or of service provision.
Knowledge of the City of Salford, its people, health and community services and its diversity. / X
