Job Title: Senior Care Worker

Salary: £18,188 per annum

Responsible to: Home Care Manager

Location: Chorley, Leyland, Ormskirk, and Preston

SUMMARY

  • Supervisory Management of a team of Care Workers
  • Direct delivery of Home Care packages
  1. SERVICE ETHOS

To provide Home Care Services within a culture and ethos which promotes independence and develops resilience. This will involve maximising service users self- care abilities, developing coping strategies, enhancing wellbeing and providing person centred services which recognise service users individuality and personal preferences.

2. LOCATION

One post will be based in West Lancashire, one post will be based in Chorley.

3. SUPERVISORY RESPONSIBILITY

Direct responsibility for managing a team of Care Workers

  1. MAIN DUTIES

Supervisory Management

  • To provide effective line management including providing regular supervision, appraisal and assessment of training needs and ensure Care Workers are supported to maintain training and qualifications in accordance with cqc requirements
  • To ensure that Care Workers carrying out regulated activities are appropriately supported and supervised including providing regular supervision and day to day support and direction to deliver services safely and to an appropriate standard.
  • To have knowledge of the Health and Social Care Act (2008) and be responsible for completing statutory notifications for the Care Quality Commission.
  • To ensure person centred plans are established, implemented and reviewed and risk assessments and risk management plans are undertaken and reviewed in accordance with policy.
  • To ensure support plans are personalised ensuring service users are given the opportunity to exercise meaningful choice and control.
  • To ensure staff follow Imagine’s Safeguarding procedure which in place to safeguard and protect people from abuse
  • To work with families and other services to ensure they understand the ethos of maximizing independence
  • To deliver care via support planning, Communication Passports and Assistive Technology
  • To comply with the EMS (Electronic Management System)
  • To be proactive in accepting referrals through effective management of referrals, workforce capacity and staff rostering/coordination
  • To report to the Care Navigation Service on a weekly basis to confirm the availability and capacity of the Service.
  • To provide a response to Home Care Packages requests to the Care Navigation Service via Oracle Sourcing System within required timescales.
  • To ensure there are adequate resources to provide cover seven days per week including bank holidays including cover for sickness, annual leave and to respond flexibly when dealing with fluctuating needs.
  • To ensure support and care is delivered in accordance with support plans. To ensure Care Workers keep accurate up to date records
  • To ensure partnership work is established with key agencies
  • To ensure financial procedures are followed on time and in an accurate manner.
  • To ensure teams work towards clearly defined targets, and databases are maintained.
  • To involve service users in recruitment & selection, development of services, reviewing of policies and procedures and encouraged service users to participate in service user forums
  • To be actively involved in tendering for mini competitions through the Framework
  • To ensure services are fully compliant with Imagine’s Quality Management System
  • To work as part of a team providing cover for services including out of hours management arrangements.
  • To ensure that all Imagine policies and procedures are implemented and that ISO standards are maintained at all times
  • To develop appropriate local networks and represent Imagine within relevant forums ensuring that a positive and consistent image of the organisation is maintained
  • To ensure Care Workers utilise the Electronic Time Management System and use data to check performance against contractual requirements

Direct delivery of support

  • To provide person centred support using support planning tools, review and record the achievements of and progress towards goals including providing direct assistance with or regular encouragement to perform tasks of daily living
  • Establish and review risk assessments and report any changes in service users presentations
  • To promote the independence of service users by adopting an enabling approach including supporting service users to gain/regain skills relating to personal care and if required perform domestic tasks such as shopping, cooking, cleaning and budgeting
  • Support service users to access community resources and encourage best use of assistive technology
  • Support service users to develop problem solving skills and coping strategies
  • To support service users to develop confidence and engage in socially inclusive opportunities including sports and leisure; training and education; volunteering and employment opportunities
  • To support service users to improve their health and wellbeing, complete physical health checklists, enable access to health care services with a view to improving physical health
  • Support service users to improve and manage their mental health needs
  • Work in partnership with service users and care co-ordinators, social workers and drug and alcohol services to assist service users to achieve personal goals and improve self- management.
  • To ensure records are kept up to date including completing medication records, daily logs, reviewing and updating support plans and risk assessments, completing incident reports.
  • To work with families and other services to ensure they understand the ethos of maximizing independence.
  • To deliver care via Support Planning, Communication Passports and Assistive Technology
