JOB TITLE: Relief - Flexi Support Worker (Baschurch)

ACCOUNTABLE TO: Community Support Manager

SALARY: £7.20 per hour plus 86p per hour worked in lieu of holiday entitlement

KEY PURPOSE OF ROLE:

To consistently deliver a person centered service for people with a

learning disability, ensuring opportunities to develop their skills and actively participate within their home and community.

RESPONSIBILITIES:

·  Ensure your own actions support the care, protection and well-being of the individuals you support.

·  Support individuals to meet their personal and domestic needs.

·  Support individuals to access community activities

·  Support individuals to take part in development activities

·  Communicate with and complete records for individuals supported

·  Support individuals using the person centered plans, care plans and risk assessments in place.

·  Interact and relate to individuals supported

·  Support individuals to maintain a safe and clean environment

·  Support individuals in taking their medication

·  Receive and pass on messages and information

·  Support individuals with finances in line with individual plans

·  Ensure your own actions support the equality, diversity, rights and responsibilities of individuals supported

·  Contribute to the effectiveness of your team

·  Undertake statutory training required for the role and any other development opportunities identified by your line manager, in accordance with Perthyn delivery arrangements

·  Attend the necessary number of formal supervision meetings to comply with care standards requirements

·  Undertake any other duties commensurate with your grade.

Experience

Experience of working with people with learning disabilities / D
Experience of working within a health or social care environment / D
Experience of working with people who challenge services / D

Qualifications

QCF Level 2 Care / D
Certificate in Working With People With Learning Disabilities (LDAF) Level 2 / D
