Job Title:Recovery Support Worker

Reporting to:Service Manager

Location:Windsor and Maidenhead

Salary:£17,800 - £20,447

Working Hours:37 hours (including some evening work and some Saturdays)

Key internal:Service Manager; Team Leader, Recovery workers,

Relations Administrators, volunteers and nurse

Key external:Clients and their families; Partner agencies

Main purpose of the job

To provide recovery support services to service users and front line workers.

Main Duties and Responsibilities:

Working with Clients

  • Establishing strong working relationships with Service Users
  • Carrying out assessments, identification of priorities and ongoing need
  • Carrying out risk assessment and management
  • Developing a Recovery Plan with the service user, setting short, medium and long term goals pertaining to 'Recovery beyond treatment'
  • Providing information and advice, including harm minimisation and family support, as appropriate
  • Providing Structured Interventions that support the individual in meeting their goals
  • Support service user’s to achieve their recovery goals agreed around housing, benefits, education, training and employment within their Recovery Plan as identified by the service user and the Recovery Worker
  • Ensure that the appropriate service user paperwork is completed accurate and within SMART’s guidelines and systems; data is submitted within agreed time limit and all client records are accurate and up to-date
  • Maintain client records on database systems (Illy)
  • Conduct home visits when required
  • Support the facilitation of group work sessions with service users or families and careers including evening groups when appropriate
  • Understand and promote Mutual Aid and Peer Support
  • To develop and maintain local business networks to support recovery opportunities for service user.

Service Support

  • Share relevant and appropriate information within a multi-disciplinary team, to ensure continuity of care. This includes the attending and contributing to multi-agency care coordination meetings i.e. safeguarding meetings
  • Make appropriate referrals to other agencies as appropriate
  • Carry out duties of the post in accordance with SMART’s diversity and equal opportunities policy.

Other

  • To keep abreast of new developments in the field of substance misuse and criminal justice system by participating in debate and discussions in the field
  • To participate in SMART’s performance management processes
  • To carry out any other duties and responsibilities as required in line with operational needs
  • Understand and promote the values, vision and mission of SMART by effective implementation of SMART’s policies and procedures.

This job description will be supplemented by annual objectives which will be developed in conjunction with the post holder. It will be subject to regular review and SMART reserves the right to amend or add to the duties listed above.
